Registered Nurse - Home Care Client Assessments

About the Role:
Join Rising Star Home Care Services as a Registered Nurse (RN) specializing in Home Care Client Assessments. This per diem position offers the flexibility to provide compassionate care in the comfort of our clients' homes in Elkton, MD.

Responsibilities:

• Conduct comprehensive assessments of clients' health and home care needs.
• Develop and implement individualized care plans in collaboration with clients and families.
• Monitor patient progress and adjust care plans as necessary.
• Provide education and support to clients and their families regarding health management.
• Document assessments, care plans, and patient interactions accurately and timely.
• Coordinate with other healthcare professionals to ensure quality care delivery.
• Adhere to all safety and health regulations while providing care.
• Participate in ongoing training and development opportunities.

Requirements:

• Current RN license in the state of Maryland.
• Minimum of 1 year of nursing experience, preferably in home care or community health.
• Strong assessment and critical thinking skills.
• Excellent communication and interpersonal skills.
• Ability to work independently and manage time effectively.
• Compassionate demeanor with a passion for patient care.
• Current CPR and First Aid certifications.
• Reliable transportation for travel to client locations.
