Make a Difference Beyond the Clinic

Join our team as a Registered Nurse and provide meaningful, community-based care to adults living with mental illness. You’ll work alongside an integrated treatment team to support clients’ physical and mental health through nursing care, medication management, education, and care coordination.

What You’ll Do

Provide holistic nursing care in clients’ homes, residences, and community settings.
Assess physical and mental health needs and develop individualized care plans.
Provide medication administration and support clients with medication management.
Coordinate medication changes and reviews with physicians, pharmacies, and treatment teams.
Communicate with hospitals and long-term care facilities to support smooth transitions and discharge planning.
Monitor and communicate changes in clients’ health, symptoms, medications, and behaviors.
Provide health education and consultation to clients and program staff.
Promote wellness and connect clients with appropriate medical care and community resources.
Maintain accurate, timely clinical documentation and medication records.
Collaborate with an integrated team to provide coordinated, client-centered care.
Provide transportation to clients in your personal vehicle as needed.
Participate in team activities and other duties as assigned.

Qualifications:
Required Education, Experience, Certifications, Licensure and Credentials: (Where appropriate, education and/or experience may be substituted)

Minimum Required Education: Associate’s degree is required in the field of Nursing.

Minimum Required Experience: Must have at least 1,000 hours of clinical experience working with working with adults with severe and persistent mental illness.

Required License – Certification – Registration: Current CPR certification or obtained  within  the  first 90 days of employment: Medical Terminology.

Travel Type: Up to 25%

Required Valid Driver’s License: Yes

Required Auto Insurance: Motor vehicle record acceptable to program and/ or contractual requirements. A valid Wisconsin driver's license, and automobile insurance with minimum limits of

$50,000/$100,000 bodily injury and $10,000 property damage.

Knowledge, Skills, & Abilities:

Knowledge of mental illness, substance use disorders, psychopharmacology and evidenced based practices specific to needs of the clients.
Awareness of personal, cultural, educational, and environmental aspects of community living for individuals living with a mental illness or substance use disorder.
Knowledge of biological, psychological, and social problems and appropriate therapeutic interventions.
Knowledge of psychotropic medications and side effects.
Knowledge of community health, social service, and advocacy resources available within the community.
Must be familiar with Microsoft Word, Outlook, Teams, and Excel.
Professional workplace appearance and conduct; friendliness and courtesy to consumers, coworkers, and vendors.
Knowledge of community support programs in Milwaukee County is desirable.
Ability to communicate clearly and concisely in English, both orally and in writing. 
Familiarity with the Electronic Health Records (EHRs) system is desirable.
Access to a personal vehicle.
This position must be able to effectively communicate complex ideas and instructions to a variety of people from different backgrounds in both writing and verbally.

Physical Requirements, Visual Acuity, and Work Conditions:

Physical Requirements: On occasion must be able to push/pull/lift/move objects weighing at least 20 pounds. Must have the ability to climb at least two flights of stairs. Work is performed largely in the community but may at times include an office environment or other various settings.  

Visual Acuity: The worker is required to have close visual acuity to perform an activity such as: preparing and analyzing data and figures; transcribing; viewing a computer terminal.

Working Conditions: This is a community-based position. Staff are expected to utilize their personal vehicles to transport clients and meet with clients in their homes. This position may work in a variety of indoor and outdoor work environments which may include various outdoor weather conditions, loud noises, fluctuating temperatures and the occasional use of stairs.  

The hours of work will generally be during regular business hours and average at least 40 hours a week.
