Position Summary

The Referral Coordinator is responsible for managing referrals and admissions processes, coordinating communication between referral sources and clinical staff, and ensuring that all required documentation is received and accurately entered into organizational systems. This position plays a critical role in providing exceptional customer service and ensuring patients and families receive timely access to care.

Essential Responsibilities

• Answer incoming referral telephone calls and respond to electronic referrals, emails, and faxes.
• Provide information regarding Snowline Health services to physicians, patients, family members, facilities, and community partners.
• Receive and process referral information accurately and efficiently.
• Schedule patient evaluations and coordinate appointments with admission nurses.
• Communicate referral details and pertinent information to clinical staff.
• Request and obtain required medical records and supporting documentation from referral sources.
• Enter referral and admission information into the Electronic Medical Record (EMR) system.
• Coordinate admission packets and documentation with admission nurses.
• Monitor and maintain patient census information.
• Collaborate with the marketing team to ensure facility agreements, waivers, and contracts are in place prior to admission.
• Coordinate durable medical equipment (DME) needs during the referral and admission process.
• Prepare and distribute Kaiser Diverted Patient Reports and maintain required records.
• Ensure all intake and admission documentation is received, uploaded, and available for clinical teams.
• Communicate and relay messages promptly, accurately, and professionally.
• Collaborate with admissions and interdisciplinary teams to ensure smooth transitions into care.
• Complete data entry across multiple admission platforms with accuracy and timeliness.
• Deliver outstanding customer service to patients, families, physicians, staff, and referral partners.
• Maintain confidentiality and ensure compliance with HIPAA regulations.
• Demonstrate initiative, accountability, adaptability, and strong teamwork skills.
• Respect and support individuals with diverse backgrounds, beliefs, lifestyles, and preferences.
• Perform responsibilities efficiently, accurately, and within established timelines.
• Complete other duties as assigned.

Requirements

Qualifications:

• Referral Coordination Experience Required
• One year experience in a healthcare setting preferred
• LVN experience preferred 
• High School Graduate or GED
• Four years general office and administrative support experience OR two years general office and administrative support plus an Associate Degree in business or related field
• Knowledge of medical terminology
• Demonstrated ability to perform detailed work in a timely manner with accuracy and thoroughness
• Demonstrated ability to prioritize tasks, handle pressure and delicate situations while effectively carrying out job responsibilities
• Demonstrated computer skills with extensive experience using Microsoft Word and Excel required, Access preferred
• Electronic Medical Records (EMR) experience preferred
• Must be able to rotate weekends with the rest of the team
