Job Description
Summary: The Re-engagement Specialist is responsible for conducting in-person outreach to clients who have been disengaged from services for more than thirty days. The primary objective of this role is to re-engage these clients and encourage them to resume utilizing the services provided by Preventive Measures. The Re-engagement Specialist will work closely with the client, providing support, information, and assistance to address any barriers preventing re-engagement. This position requires excellent communication skills, empathy, and a proactive approach to problem-solving.
Interrelationship: Maintain positive, cooperative, and mutually supportive relationships with your supervisor, the administrative, peer, and support staff. Establish a professional rapport with clients Demonstrate initiative, enthusiasm, and positive interpersonal skills to relate well with clients and staff.
Primary Job Responsibilities:
1. Conduct in-person visits to clients' homes who have been disengaged from services for more than thirty days.
2. Establish rapport and build trust with clients through active listening and empathetic communication.
3. Assess the reasons for disengagement and identify any barriers preventing re-engagement.
4. Provide information about available services, benefits, and resources to encourage clients to resume utilizing the organization's offerings.
5. Collaborate with internal teams to address any specific concerns or issues raised by clients during the outreach process.
6. Develop personalized re-engagement plans for each client, outlining steps and strategies to overcome barriers and facilitate their return to services.
7. Maintain accurate documentation of all interactions, including progress notes, outcomes, and follow up actions.
8. Monitor and track client progress towards re-engagement goals, providing ongoing support as needed.
9. Collaborate with community partners and stakeholders to leverage additional resources that may assist in the re-engagement process.
10. Stay updated on industry trends, best practices, and relevant regulations related to client engagement.
Job Skills:
- High school diploma or equivalent;
- Previous experience in customer service, outreach, or community engagement roles is highly desirable.
- Excellent interpersonal and communication skills, with the ability to engage effectively with diverse populations.
- Strong problem-solving and decision-making abilities.
- Empathy and compassion towards individuals facing challenges or barriers to accessing services.
Proficiency in using technology for documentation and communication purposes.
- Valid driver's license and access to reliable transportation.
Education/Experience:
- High school diploma or equivalent;
