Quality and Outreach Specialist

Job Details: Job Location: Via Care - 501 - Los Angeles, CA, Salary Range: $25.00 - $28.00Hourly, ESSENTIAL DUTIES AND RESPONSIBILITIES

Provide enrollment assistance (including but not limited to completing coverage applications, gathering the required documentation and troubleshooting the enrollment process) for uninsured adults to access subsidized, low-cost and free health insurance programs through the health insurance marketplace;
Provide structured patient education on health coverage, engage in follow-up conversations and offer renewal assistance for enrolled individuals;
Distribute outreach materials to patients, community members, partner organizations and businesses to build coverage option awareness;
Develop, discover and attend community events in order to promote coverage options and the mission and services of the organization;
Collaborate with various local organizations to build awareness of coverage options, spur enrollment and build referral linkages;
Attend and successfully complete all required training programs; participate in ongoing conference calls, webinars, and other professional development opportunities;
Accurately provide required reporting to track goal achievement and client satisfaction.
Recruit and utilize volunteers to increase program capacity;
Assist in the development and implementation of organizational outreach and enrollment initiatives;
Adhere to all state and federal privacy regulations, including HIPAA and 42 CFR Part 2, and to all policies and agreements regarding confidentiality, privacy, and security. Support compliance with all privacy and security requirements pursuant to community partners’ and outside providers’ patient confidentiality agreements, including privacy and security requirements for EMR access
Assist with translation when necessary.
Assist when necessary with back-office functions.
Conduct classes, trainings, and meetings in the community, as requested; and
Other duties may be assigned or may be modified as business needs dictate.
Supporting Closing the Organizational Quality Gap (Cozeva, Innovacer,...)
Senior Care quality tracking
Support and empanelment improvement
Maintain and organize quality-related documentation, reports, and records.
Input, update, and retrieve data from quality management databases or tracking systems.
Assist in preparing quality performance reports, presentations, and summaries.
Support audits, surveys, and inspections by regulatory agencies 
Schedule quality meetings, prepare meeting minutes, and follow up on action items.
Maintain confidentiality of sensitive patient and organizational information.
Assist in disseminating policies, procedures, and training materials related to quality standards.
Coordinate communication between departments to ensure timely data submission and compliance.
Perform other clerical duties such as filing, photocopying, and correspondence as needed.

Additional Job Functions 

Senior - Annual Wellness Exams & Medicare Updates: Provide biweekly summary reports on senior annual wellness appointments, including completion rates and any relevant Medicare health plan changes, patient drops.
Supplemental Data & Cross-Referencing: Receive Cozeva supplemental data uploads biweekly and cross-reference completion by IPA.
Managed Care Weekly Reporting: Provide biweekly report on specific inventive-based outreach initiatives, detailing progress, barriers, performance metrics, and any follow-up actions needed.
IPA & Health Plan Meeting Participation: Attend IPA/health plan meetings as needed, at the discretion of the Chief Innovation & Growth Officer and CMIO.
HEDIS & Cozeva Reporting: Provide biweekly HEDIS completion reports by IPA and health plan, and biweekly summaries of all supplemental data uploads to Cozeva.
Community Engagement: Participate in senior community events and Live Well with Via Care workshops (field and in-person).

Education & Experience            

Two years’ related experience within human services and/or public health;
Must have excellent reading and comprehension skills.
Must be able to work efficiently in a very fast-paced environment
Must adhere to the agency’s non-discrimination policies.
Experience in community organizing;
Experience working in underserved communities such as undocumented workers, unaccompanied minors, LGBT populations, communities of color, youth/adolescents, and those experiencing homelessness, substance abuse, and/or mental illness; and Bilingual English/Spanish required (read, write, speak)
