What You'll Do: 

• Conduct nutrition assessments and consultations
• Develop individualized nutrition care plans
• Monitor nutritional status and recommend interventions
• Collaborate with healthcare professionals
• Support quality improvement and regulatory readiness activities Improve patient outcomes using Food as Medicine
