Scope of Position: The Neighborhood Navigator Primary Care Nurse Practitioner is a key member of an interdisciplinary, community-based team serving adults experiencing serious mental illness, substance use disorders, co-occurring medical conditions, and homelessness or street homelessness. The Nurse Practitioner provides accessible, compassionate, and trauma-informed primary care to individuals in the settings where they live and spend their time. The program serves high-need neighborhoods throughout Manhattan, including Washington Heights/Inwood, Central and East Harlem, the Lower East Side/Chinatown, and Midtown West/Hell’s Kitchen/Chelsea. Approximately 80% of this position is community-based. The Nurse Practitioner will build trusting relationships with program participants, provide basic medical care and assessments, prescribe and manage medications, and coordinate referrals for ongoing care. The role requires a strong commitment to meeting individuals “where they are” and working collaboratively with community providers and interdisciplinary team members.

This is a field-based position and requires 80% of services to be provided in the community.

Essential Position Functions:

Provide primary and basic medical care to clients in community-based and field settings.
Conduct comprehensive medical assessments, evaluations, and ongoing health monitoring.
Diagnose, treat, and manage acute and chronic medical conditions within the Nurse Practitioner’s scope of practice.
Prescribe, administer, and monitor medications in accordance with applicable laws, regulations, and organizational policies.
Provide and prescribe medication-assisted treatment (MAT) for individuals with opioid use disorder, as appropriate and within the scope of practice.
Assess and monitor clients’ physical health and identify medical conditions requiring additional evaluation or treatment.
Provide crisis intervention and clinical support during regular business hours.
Develop trusting relationships with individuals who may have experienced homelessness, serious mental illness, substance use disorders, trauma, and barriers to accessing traditional health care services.
Coordinate referrals and connections to primary care clinics, specialty providers, hospitals, behavioral health services, substance use treatment programs, and other community resources.
Collaborate with family members, housing providers, shelter staff, hospitals, criminal justice personnel, primary care providers, and other treatment partners, as appropriate.
Participate in interdisciplinary team meetings, clinical case conferences, high-risk case reviews, and care coordination activities.
Contribute to quality improvement initiatives, evidence-based practice implementation, and the development of clinical policies and procedures.
Maintain timely, accurate, and complete clinical documentation in the Electronic Health Record (EHR) in compliance with payer, funder, organizational, and regulatory requirements.
Complete required administrative documentation and reports.
Promote a person-centered, harm-reduction, trauma-informed, and culturally responsive approach to care.
Work with clients in other Bridge programs, as needed.

Qualifications
Current licensure as a Nurse Practitioner in New York State.
National board certification in an appropriate Nurse Practitioner specialty.
Ability to practice independently without a collaborating physician.
Appropriate clinical experience in primary care, community health, behavioral health, substance use treatment, or a related setting.
Ability to assess, diagnose, treat, and manage common acute and chronic medical conditions.
Experience prescribing and managing medications within the scope of practice.
Knowledge of or experience treating individuals with substance use disorders, including opioid use disorder and medication-assisted treatment.
Strong clinical assessment, communication, documentation, and care coordination skills.
Ability to work independently in community- and field-based settings while collaborating effectively with an interdisciplinary team.
Commitment to serving individuals with complex medical, behavioral health, and social needs.
Experience working with individuals experiencing homelessness or housing instability.
Experience working with individuals with serious mental illness and co-occurring substance use disorders.
Experience providing community-based, street-based, mobile, or outreach health care services.
Bilingual English/Spanish skills are highly desirable.
Familiarity with harm reduction, trauma-informed care, and culturally responsive practices.
