Summary

The provider delivers comprehensive, patient-centered primary medical care to individuals across the lifespan in community and school-based settings. They work collaboratively with an interdisciplinary team to ensure accessible, evidence-based, and culturally responsive healthcare services.

Job Description

Description

HealthFinders Collaborative (HFC) is a growing community health organization providing primary medical, dental, and behavioral healthcare, advocacy, and wellness education for people who experience barriers to care. Our holistic, community-based model is built on trust, relationships, and the belief that everyone should have access to the healthcare they need.

We are seeking a compassionate, skilled, and collaborative Primary Care Provider to join our team and provide comprehensive care to patients across the lifespan.

Position Overview

The Primary Care Provider delivers high-quality, patient-centered medical care to children, adolescents, adults, and older adults. The provider works closely with clinical leadership and an interdisciplinary team to ensure that care is accessible, culturally responsive, evidence-based, and consistent with HFC’s mission and clinical standards.

HFC’s team includes medical, dental, and behavioral health providers, community health workers, support staff, volunteers, students, and community partners. Our staff bring diverse cultures, languages, lived experiences, and professional backgrounds to their work. Many team members are also growing in their careers and developing new skills and responsibilities.

Community health work does not always follow a predictable path. Patient needs and clinic priorities can change quickly, and the right provider will bring creativity, flexibility, compassion, and sound clinical judgment to the work. Successful providers at HFC value the contributions of colleagues in every role, communicate clearly, and help the team find practical ways forward when circumstances are complex.

Position and Schedule

This position is available on either a part-time or full-time basis, as well as school-year calendar if desired. The specific schedule will be developed based on organizational needs, the provider’s availability, and the agreed-upon FTE.

The provider’s schedule may include time in HFC’s community clinics, school-based health settings, or a combination of locations. HFC will work with the selected candidate to establish a schedule that supports continuity of care, team collaboration, and reliable access for patients.

Compensation

HFC’s starting compensation ranges reflect the different professional qualifications and scopes of practice that may meet the requirements of this position:

  • Advanced-practice provider: $114,000–$150,000 annually at 1.0 FTE.
  • Physician: $200,000–$240,000 annually at 1.0 FTE.

This position is available on either a part-time or full-time basis. Compensation will be prorated based on the agreed-upon FTE. Starting compensation within the applicable range will reflect relevant experience, qualifications, scope of practice, assigned responsibilities, and internal equity. School-based and community-clinic responsibilities may both be included within the provider’s regular schedule and compensation.

Benefits

HFC offers a competitive benefits package for eligible employees, including medical and dental insurance, 403b match, PTO and paid holidays, continuing education/licensure support, and malpractice coverage. Benefit eligibility and employer contributions may vary based on the employee’s scheduled FTE.

Direct Patient Care

  • Provide comprehensive primary care to patients across the lifespan in accordance with HFC protocols, evidence-based practice, and current standards of care.
  • Provide preventive care, health maintenance, acute and episodic care, chronic disease management, reproductive healthcare, and patient education within the provider’s scope of practice.
  • Assess, diagnose, treat, and monitor patients, referring them for specialty or higher-level care when appropriate.
  • Provide clinical follow-up and coverage for HFC patients, including prescription refills, laboratory orders and results, abnormal findings, and urgent patient concerns.
  • Support continuity of care for patients seen by volunteer or other HFC providers.
  • Develop care plans that consider the clinical, cultural, social, and economic factors affecting each patient’s health and ability to access care.
  • Maintain timely, accurate, and complete documentation in the electronic health record.
  • Complete workers’ compensation, insurance, disability, and other patient-related forms as appropriate.
  • Participate in pre-visit planning, care-team huddles, case consultation, and coordination of patient care.
  • Communicate with patients through the patient portal, telephone, secure messaging, or other appropriate methods.
  • Work collaboratively with other HFC providers, staff, contractors, volunteers, and external partners to support coordinated care.

Team and Clinical Collaboration

  • Provide clinical consultation and support to other providers, volunteers, community health workers, and members of the care team.
  • Offer clear and constructive clinical guidance while supporting staff and volunteers who are developing their skills and growing in their careers.
  • Recognize and respect the knowledge, experience, and contributions that each team member brings to patient care.
  • Collaborate across medical, dental, behavioral health, care coordination, and community-based services.
  • Support clinical learning experiences for students, volunteers, and team members.
  • Provide community and patient education as requested.
  • Help build a clinic culture that values learning, accountability, teamwork, and continuous improvement.

Quality and Clinic Operations

  • Participate in quality-improvement initiatives that strengthen clinical care, patient safety, workflows, protocols, and health outcomes.
  • Help review, develop, and implement clinical policies and procedures consistent with regulatory requirements and accepted standards of practice.
  • Support the ongoing training and development of community health workers, clinical support staff, and other members of the health team.
  • Participate in the review and resolution of patient concerns or formal complaints when clinical input is needed.
  • Support HFC’s response to occupational exposures or other staff and volunteer health concerns when requested and appropriate.
  • Follow infection-prevention, workplace-safety, confidentiality, and compliance requirements.
  • Maintain the continuing education, certification, and licensure requirements necessary to remain in good standing.
  • Contribute to other clinical and organizational priorities reasonably related to the position and HFC’s mission.

About HealthFinders Collaborative

HFC works to ensure that everyone has access to the healthcare they need. We believe quality care begins with trust and is strongest when medical, dental, behavioral health, and community supports work together. Our team is committed to meeting people where they are, reducing barriers to care, and building healthier communities through relationships that are both compassionate and accountable.

HealthFinders Collaborative is an equal opportunity employer. We value the perspectives and experiences of a diverse workforce and encourage applications from candidates whose backgrounds reflect the communities we serve.

  • Education, certification, training, and demonstrated clinical competence to care for patients across the lifespan, including pediatric and adult patients.
  • Ability to assess, diagnose, treat, prescribe, and manage preventive, acute, and chronic health conditions within the provider’s legal scope of practice.
  • Eligibility for credentialing and enrollment with HFC’s applicable healthcare payers.
  • Current BLS certification.
  • Current DEA registration, or eligibility to obtain registration before beginning responsibilities that require it.
  • Strong knowledge of physical assessment, differential diagnosis, pathophysiology, pharmacology, preventive care, and the management of acute and chronic health conditions.
  • Ability to maintain timely and accurate clinical documentation and meet applicable regulatory, reimbursement, and quality requirements.
  • Strong organizational skills and the ability to prioritize, respond appropriately to urgent needs, and manage multiple responsibilities.
  • Ability to use an electronic health record, patient portal, email, and other technology needed to carry out the position’s responsibilities.
  • Sufficient English-language proficiency to communicate effectively with patients, staff, and partners and to complete clinical documentation.

  • Current board certification or board eligibility appropriate to the position’s scope of primary care practice.
  • At least two years of experience providing primary care across the lifespan.
  • Experience in community health, a safety-net setting, or another resource-limited clinical environment.
  • Experience serving patients who are uninsured or underinsured, immigrants or refugees, or who experience other barriers to healthcare.
  • Experience working with interpreters and providing linguistically and culturally responsive care.
  • Experience working within an interdisciplinary healthcare team.
  • Spanish, Somali, or another language commonly spoken by HFC patients is valued but not required.

What Will Help Someone Thrive at HFC

The successful candidate will:

  • Approach patients and colleagues with compassion, curiosity, humility, and respect.
  • Demonstrate flexibility and creativity in responding to changing patient, clinic, and community needs.
  • Remain calm, resourceful, and clinically thoughtful when circumstances are complex or priorities shift.
  • Work collaboratively across professional roles without losing sight of clinical responsibility and patient safety.
  • Enjoy teaching, coaching, and supporting colleagues who are developing their skills.
  • Communicate expectations, concerns, and clinical guidance clearly and constructively.
  • Practice with cultural humility and adapt care to each patient’s background, experiences, values, and needs.
  • Balance independence and initiative with a willingness to consult, collaborate, and ask for help.
  • Follow through reliably on patient care and organizational responsibilities.
  • Demonstrate a genuine commitment to HFC’s mission, health equity, and community-based care.
