Job Details: Job Location: Malone Office - Malone, NY, Position Type: Full-Time, Salary Range: $23.03 - $24.04Hourly, Travel Percentage: Up to 50% , Job Shift: Day, Help young people and families move forward—together.

As a Family Specialist at Together for Youth, you’ll work directly with youth and families during times when the right support can make a real difference. You’ll build relationships, help families identify their strengths and goals, connect them with resources, and work alongside them toward greater stability and a stronger future.

This is a role for someone who wants more than a desk job. You’ll spend time with youth and families in their homes and communities, collaborate with schools and other partners, solve problems, celebrate progress, and help families navigate challenges along the way.
Description: What You’ll Do

Build relationships that matter. Get to know youth and families, understand what’s important to them, and create supportive, caring relationships grounded in trust and respect.
Help families build on their strengths. Partner with families to identify needs, set goals, overcome barriers, and develop plans that support meaningful and lasting progress.
Be part of the plan. Work with youth, families, colleagues, and community partners to develop, implement, and continually evaluate individualized service and permanency plans.
Show up where support is needed. Meet regularly with youth and families in their homes and communities and help them work toward their goals in real-life settings.
Connect the dots. Coordinate services and help ensure youth have access to medical care, dental care, education, counseling, and other resources they need to thrive.
Bring people together. Build strong working relationships with schools, service providers, community organizations, and others who are part of a young person’s support system.
Help navigate what comes next. Support youth and families in preparing for successful transitions, including reunification, permanency, or other appropriate plans when returning home is not possible.
Keep the team informed. Maintain accurate, timely documentation and ensure services meet agency, regulatory, confidentiality, and consent requirements.
Be there when things get difficult. Respond calmly and thoughtfully to challenges and crises, helping youth and families identify solutions and next steps.

What You’ll Bring

A bachelor’s degree in human services or a related field.
At least one year of experience working with youth and families and providing case management or similar services.
Strong communication skills—you know how to listen, build trust, have difficult conversations, and communicate clearly.
The ability to stay grounded, flexible, and solution-focused when situations become challenging.
Respect for people with different backgrounds, experiences, identities, cultures, and perspectives.
A willingness to work a flexible schedule based on the needs of youth and families, which may include some evenings, weekends, holidays, and participation in an on-call rotation.
A valid driver’s license, driving record acceptable to Together for Youth, and reliable transportation. Travel within the community is an important part of this position.
Bilingual skills are a plus.
