Job Description
Elite is seeking compassionate and dedicated Physical Therapy Assistants to join our team in providing exceptional home health and hospice care. What You'll Do:

Provide physical therapy services under the direction of a licensed Physical Therapist

Follow individualized care plans and document patient progress

Educate patients and caregivers on exercises and safety techniques

Collaborate with an interdisciplinary team to ensure the best outcomes

Travel to patient homes within the assigned service area
What You'll Bring:
