About the role
Sylva Valley Health & Rehab is hiring a Physical Therapist (PT) and/or Physical Therapist Assistant (PTA) to provide high-quality rehab services in a skilled nursing setting. This role is ideal for clinicians who value patient-centered care, strong clinical outcomes, and teamwork with nursing and the interdisciplinary team.

Key responsibilities (PT)
- Evaluate residents and develop individualized plan of care
- Provide PT treatment to improve mobility, strength, balance, gait, and safety
- Document thoroughly to support skilled coverage and clinical progress
- Coordinate with OT, SLP, nursing, and providers regarding changes in condition
- Educate residents and families on exercises, safety, and discharge planning
- Participate in care conferences/IDT as needed
Key responsibilities (PTA)
- Provide treatment interventions under the supervision of a PT
- Document treatments and resident response/progress
- Communicate changes in condition to the supervising PT and care team
- Assist with resident and family education per plan of care
- Support discharge planning and continuity of care
Requirements
- PT: Current Physical Therapist license + graduate of an accredited PT program
- PTA: Current Physical Therapist Assistant license + graduate of an accredited PTA program
- Strong documentation and communication skills
- SNF/rehab experience preferred (new grads considered)
