Position Summary

GENISCi Health & Wellness is seeking qualified Physical Therapists (PT) and/or Occupational Therapists (OT) who value professional accountability, clinical teamwork, accurate documentation, and respectful client-centered care. Physical Therapist or Occupational Therapist will provide functional assessments, movement evaluations, rehabilitation guidance, and mobility-focused interventions. They will work collaboratively with interdisciplinary team members to help individuals improve physical function, reduce limitations, prevent injury, and enhance overall well-being.

Key Responsibilities

• Conduct comprehensive physical or occupation therapy evaluations and functional assessments.
• Develop individualized treatment and rehabilitation plans based on patient goals and clinical findings.
• Implement therapeutic exercises, mobility training, balance training, and injury prevention strategies.
• Educate patients and caregivers on exercises, mobility techniques, safety practices, and wellness strategies.
• Monitor patient progress and modify treatment plans as appropriate.
• Collaborate with physicians, nurses, care coordinators, wellness professionals, and other healthcare team members.
• Maintain timely and accurate clinical documentation.
• Participate in interdisciplinary care planning and case reviews.
• Support community wellness initiatives, educational programs, and preventive health activities.
• Ensure compliance with applicable state and federal regulations, organizational policies, and professional standards.

Qualifications

Required

• Doctor of Physical Therapy (DPT), Master of Occupational Therapy (MOT) or Doctor of Occupational Therapy (OTD), or equivalent degree from an accredited program.
• Current active PT / OT license in the Commonwealth of Virginia.
• Home/place-of-care assessment.
• Fall-risk and safety evaluation, functional mobility, DME/adaptive-equipment recommendations.
• Discharge/transition planning, escalation of material clinical changes.
• Coordination with the RN/DON and ordering provider, and compliance with the individualized plan of care.
• Excellent communication and interpersonal abilities.
• Ability to work independently and collaboratively within a multidisciplinary environment.
• Computer proficiency and experience with electronic documentation systems.
• Must pass background check, TB screening, and meet all VDH personnel qualifications for PT/OT position.

Preferred

• Experience in outpatient rehabilitation, home health, wellness, preventive care, or integrative health settings.
• Experience working with older adults and individuals with chronic conditions.
• Certification in specialized rehabilitation areas or advanced therapeutic techniques.
• Experience providing patient education and wellness coaching.

Must be able to stand, walk, squat, bend, kneel, reach, twist, push and pull.

Must be able to lift up to 50 pounds.
