Home Health Physical Therapist (PT) – Full-Time / Part-Time / Weekend Only Central Coast Home Health | San Luis Obispo, California

Central Coast Home Health is a locally owned, fast-growing agency with an excellent reputation across the Central Coast. We are seeking Home Health Physical Therapists (PT) to provide high-quality, patient-centered care in multiple San Luis Obispo Areas:    North County (Atascadero/Templeton/Paso Robles)

              South County (Pismo Beach/Arroyo Grande/Nipomo/Santa Maria)

              Santa Maria-Lompoc

              Lompoc-Santa Ynez Valley

Responsibilities

• Provide in-home physical therapy services
• Develop individualized care plans
• Collaborate with interdisciplinary team members
• Document patient progress accurately and timely

Qualifications

• Active California Physical Therapist (PT) license
• CPR certification
• Valid driver’s license + clean driving record
• Reliable transportation & auto insurance
• Ability to work independently

Home health experience preferred; new grads welcome!

Pay

• Routine Visits: $69–$79
• Evaluations: $91–$104
• Admissions: $172.50–$197.50

Actual pay rates vary depending on factors including type of visit, type of experience, location, and years of experience.
