What You’ll Do: 

Provide physical therapy services in post-acute and inpatient care settings 

Conduct evaluations and develop customized treatment plans 

Help patients regain strength, balance, mobility, and independence 

Perform therapeutic interventions, including exercise, gait training, and manual therapy 

Document treatment progress and adjust plans as needed 

Collaborate with care teams to drive patient recovery and functional outcomes 

Educate patients, families, and facility staff on mobility and safety practices 

Requirements: 

Graduate of an accredited Physical Therapy program 

Active PT license in the state of WI

Minimum of one year of experience in a clinical setting such as SNF, ALF, acute care, or rehab 

CPR certification (American Heart Association or American Red Cross) 

Solid clinical judgment, communication skills, and adaptability 

Dependable, professional, and team-focused
