About the Role:AlwaysNear Home Care LLC is looking for a dedicated and compassionate Physical Therapist (PRN) to join our growing team in Atlanta, GA. Responsibilities:

• Conduct comprehensive physical therapy evaluations and assessments for home care patients
• Develop and implement individualized treatment plans to restore function and improve mobility
• Provide therapeutic interventions including therapeutic exercise, manual therapy, and neuromuscular re-education
• Monitor and document patient progress, adjusting care plans as clinically appropriate
• Educate patients and caregivers on home exercise programs and injury prevention strategies
• Collaborate with interdisciplinary home health team members to ensure coordinated patient care
• Maintain accurate and timely clinical documentation in compliance with regulatory standards

Requirements:

• Valid Georgia Physical Therapist (PT) license in good standing
• Graduate of an accredited Physical Therapy program (DPT or equivalent)
• Home health or outpatient physical therapy experience preferred
• Excellent communication, interpersonal, and patient education skills
• Reliable transportation and valid driver's license for home visit travel throughout Atlanta, GA
• Ability to work independently and manage a flexible PRN schedule
