Redefine Healthcare is looking for a dedicated, full-time Physical Therapist (PT) to join our expanding team of healthcare professionals in our Edison Office. The Physical Therapist (PT) will be responsible for evaluating, planning, and implementing individualized physical therapy treatment programs to help patients restore function, improve mobility, and relieve pain. Pay Rate:  $54-57/ hour/ experience

Hours: 40 hours per week

Essential Duties & Responsibilities

1. Patient Evaluation & Plan of Care

• Perform the initial patient examination and evaluation, including history, systems review, and objective testing/measures for MSK conditions.
• Establish diagnosis, prognosis, and an individualized plan of care with measurable, functional goals.
• Determine which components of the plan of care, if any, may be delegated to a PTA or to supportive personnel, consistent with N.J.A.C. 13:39A-7.1 through 13:39A-7.3 and the patient's clinical presentation.
• Perform periodic re-evaluations and revise the plan of care as clinically indicated or as required by payer policy (e.g., Medicare Part B progress reporting intervals).

2. Direct Treatment & Supervision

• Deliver skilled PT interventions (therapeutic exercise, manual therapy, neuromuscular re-education, modalities, gait and functional training) directly to patients as clinically appropriate.
• Provide the on-site supervision required by New Jersey law when a PTA implements delegated components of the plan of care and remains immediately available for consultation per N.J.A.C. 13:39A-7.2.
• Retain professional responsibility and accountability for all care rendered under the PT's plan of care, including care delivered by supervised personnel.
• Recognize and respond to clinical red flags (e.g., signs of DVT, fracture, cauda equina syndrome, cardiopulmonary compromise) and escalate to the referring physician or emergency services as warranted.

3. Interdisciplinary Coordination

• Communicate evaluation findings, progress, and discharge recommendations to referring physicians and other treating.
• Participate in care coordination with ASC-based surgeons for pre- and post-operative MSK rehabilitation pathways, where applicable.

4. Documentation & Compliance

• Complete timely, accurate documentation within the electronic health record for every encounter (initial evaluation, daily/progress notes, re-evaluations, discharge summary).
• Co-sign or review PTA documentation as required by New Jersey and Medicare regulations.
• Maintain documentation in accordance with Redefine Healthcare record-retention and HIPAA policies.

Knowledge, Skills & Abilities

• Working knowledge of New Jersey physical therapy practice law and Board of Physical Therapy Examiners rules governing PT/PTA supervision and delegation.
• Strong clinical reasoning across common outpatient musculoskeletal diagnoses (spine, shoulder, knee, hip, post-surgical orthopedic conditions).
• Effective written and verbal communication with patients and referring physicians.
• Familiarity with electronic health record (EHR) documentation and outpatient therapy billing.
• Cultural competence and patient-centered communication.

Physical Demands & Work Environment

• Ability to stand, bend, kneel, and lift/transfer patients (typically up to 50 lbs. with assistance/equipment as needed) throughout a full clinical shift.
• Manual dexterity for hands-on manual therapy techniques and equipment operation.
• Works in an outpatient clinical setting; may require travel between Care Center locations within the network.

Maintain Licensure or Certification:

• Active, unrestricted New Jersey Physical Therapist license issued by the State Board of Physical Therapy Examiners.
• Current CPR/BLS certification (American Heart Association or equivalent).
• Compliance with all applicable NJ Board of Physical Therapy Examiners continuing competency requirements for license renewal.
• Successful completion of Redefine Healthcare compliance training (HIPAA, corporate compliance, coding integrity) prior to independent patient contact.
• No current or pending licensure discipline, Medicare/Medicaid exclusion, or OIG exclusion-list finding.

Minimum Level of Education or Experience Required:

• Doctor of Physical Therapy (DPT) or equivalent entry-level PT degree from a CAPTE-accredited program.
• Outpatient orthopedic clinical experience preferred; new graduates considered with appropriate mentorship.
• Experience in a physician-affiliated outpatient setting is a plus
