Job Details: Job Location: CareOne Home Health LLC - Monterey, CA, Position Type: Per Diem, Salary Range: $70.00 - $80.00Hourly, Travel Percentage: Field Position, Join our growing team! CareOne Home Health is looking for a Physical Therapist for our growing Home Health business serving Monterey County and surrounding communities. Mileage is reimbursed and driving time is paid.  This is a per diem position with full time potential as we grow.

Skills/Requirements

1 year of experience as a licensed Physical Therapist
A current and unrestricted license to practice Physical Therapy in the State of California.
Current CPR/BLS certification.
Reliable transportation
Compliant with state healthcare regulatory requirements.
Possess up-to-date knowledge of treatment practices and physical therapy programs.
Ability to care for patients with different personalities.
Ability to assess patient condition and work with treatment plans.
Strong skills in observation, verbal and written communication, problem-solving, basic math, Electronic Medical Record Software (Kinnser a plus), and basic computer skills to manage email, texts, employment record self-service, etc.

Essential Job Functions/Responsibilities

Initiates physical therapy evaluation and care plan and instructs other personnel and/or family/ caregivers in certain phases of physical therapy with which they may work with a patient, focusing on patients therapeutic rehabilitation. 
Educate and prepare patient for use of prosthetic devices
Prepares and submits clinical and progress summaries utilizing OASIS for all documentation
Participates in patient start of care and discharge planning. 
Provides physical therapy consultation to families/caregivers, as needed
Provides in-service education programs for nursing personnel as needed.
Participates in the interdisciplinary group.
Actively participates in the quality assessment performance improvement teams and activities.
Provides supervision of physical therapist assistants
