 Vaiden Community Living Center is looking for a Physical Therapist to join our team and make a meaningful difference in the lives of the residents we serve. In this role, you’ll use your clinical expertise to assess resident needs, develop individualized treatment plans, and provide high-quality physical therapy in a collaborative healthcare environment.

Essential Duties: 

• Assess resident conditions and develop individualized treatment goals and plans in partnership with residents.
• Provide physical therapy treatments designed to improve function, mobility, strength, and quality of life.
• Verify required authorizations before beginning evaluations or treatment.
• Accurately document evaluations, treatments, progress, and other resident information in the medical record.
• Code therapy procedures and diagnoses accurately and in a timely manner.
• Effectively navigate and document within the electronic medical record (EMR) system.

Requirements

Requirements: 

• Graduate of an American Physical Therapy Association (APTA)-accredited Physical Therapy program.
• Current Physical Therapist license in the state of practice.
