Key Responsibilities
Treatment & Resident Care

Deliver high-quality physical therapy interventions under the direction and supervision of a licensed Physical Therapist.

Provide individualized treatment focused on mobility, strength, balance, functional independence, and quality of life.

Monitor resident responses to treatment and communicate progress or concerns to the supervising PT.
Documentation & Progress Monitoring
-
Maintain accurate and timely clinical documentation.

Track resident progress and support achievement of established treatment goals.

Follow facility policies, clinical standards, and applicable regulatory requirements.
Interdisciplinary Collaboration

Collaborate with Nursing, Activities, and the broader Therapy team to support comprehensive resident care.

Participate in care planning and communicate effectively with interdisciplinary team members.
Resident & Caregiver Education

Educate residents and caregivers on safe mobility techniques, exercises, and strategies to promote independence.

Support caregivers and staff with carryover of appropriate therapeutic and restorative techniques.
Qualifications
-
Current, valid California Physical Therapist Assistant (PTA) License required.

Strong communication and interpersonal skills.

Team-oriented mindset with a passion for working with residents in a skilled nursing and post-acute setting.
