Physical Therapist Responsibilities
- Evaluate residents and develop individualized treatment plans
- Provide evidence-based physical therapy services to improve mobility, strength, balance, and functional independence
- Assess patient needs and establish measurable treatment goals
- Monitor patient progress and modify treatment plans as appropriate
- Collaborate with physicians, nursing staff, therapy team members, and families
- Educate patients and caregivers regarding treatment plans, exercises, and safety recommendations
- Participate in interdisciplinary care planning meetings and patient conferences
- Supervise Physical Therapist Assistants (PTAs) as appropriate
- Recommend adaptive equipment and mobility aids when necessary
- Maintain accurate and timely documentation in accordance with facility, state, and federal guidelines
Qualifications
- Current California Physical Therapist (PT) License required
- Graduate of an accredited Physical Therapy program
- Strong clinical assessment and treatment planning skills
- Excellent communication and interpersonal abilities
- Ability to work effectively within an interdisciplinary healthcare team
- Skilled Nursing Facility (SNF), Post-Acute Care, Rehabilitation, or Long-Term Care experience preferred
