Job Description
We are seeking a Physical Therapist who is able to travel to Wayne, Macomb, or Oakland counties to provide high-quality care to our patients.
Responsibilities
- Provide physical therapy services to patients in their homes across Wayne, Macomb, and Oakland counties.
- Develop individualized treatment plans based on patient assessments.
- Monitor patient progress and adjust treatments as necessary.
- Maintain accurate and timely documentation of all patient interactions.
- Collaborate with other healthcare professionals to ensure comprehensive patient care.
Requirements
- Licensed Physical Therapist in the state of Michigan.
- Ability and willingness to travel to Wayne, Macomb, and Oakland counties.
- Strong communication and interpersonal skills.
- Valid driver’s license and reliable transportation.
- Experience with home health physical therapy preferred.
