Job Overview

We are seeking a skilled and dedicated Physical Therapist (PT) to join our growing home health team serving Lakeland and surrounding areas.

As a Physical Therapist with UPLIFT HOMECARE, you will provide skilled physical therapy services in patients' homes, develop individualized treatment plans, monitor progress, and collaborate with physicians and other healthcare professionals to achieve positive patient outcomes.

Responsibilities

As a Physical Therapist, you will:

• Provide skilled physical therapy services in accordance with the physician's plan of treatment and the patient's plan of care.
• Assist the physician in evaluating and assessing the patient's level of functioning through appropriate tests and clinical assessments.
• Assist in the development and revision of individualized plans of care.
• Provide appropriate therapeutic exercises, physical therapy treatments, modalities, and procedures to relieve pain, restore function, improve mobility, and maximize independence.
• Assess and regularly reevaluate patient needs and progress.
• Develop, implement, and revise individualized treatment plans as clinically appropriate.
• Observe, document, and report patient responses to treatment and changes in condition to the physician, Clinical Manager, and Case Manager.
• Educate patients, caregivers, and healthcare team members regarding physical therapy interventions, exercises, safety, mobility, prosthetic and orthotic devices, and durable medical equipment as appropriate.
• Initiate appropriate preventive and rehabilitative procedures within the scope of physical therapy practice.
• Promote patient safety, fall prevention, mobility, and functional independence.
• Coordinate care with physicians, nurses, occupational therapists, speech therapists, case managers, and other members of the interdisciplinary team.
• Participate in interdisciplinary case conferences and communicate effectively with physicians and field staff.
• Provide staff in-service education as directed by the Clinical Manager.
• Maintain accurate, timely, and complete patient care records in accordance with agency policies and applicable regulations.
• Maintain current knowledge of physical therapy practices, documentation requirements, clinical trends, and care coordination.

Qualifications

The ideal candidate will have:

• Current Physical Therapist license in the State of Florida.
• Minimum of 2 years of experience as a Physical Therapist preferred.
• Experience working with diverse patient populations, particularly geriatric and medically complex patients, preferred.
• Experience with pediatrics, developmental disabilities, hospice, or palliative care is a plus.
• Minimum 1 year of OASIS experience.
• AHCA Level II Background Screening.
• Current CPR certification.
• Valid driver's license.
• Current auto insurance.
• Reliable transportation.
• Strong clinical judgment and assessment skills.
• Excellent communication and interpersonal skills.
• Ability to work independently while collaborating effectively with an interdisciplinary care team.
• Compassionate, dependable, resourceful, and patient-centered approach.
