Are you a pharmacist who enjoys building relationships with patients, collaborating with a multidisciplinary care team, and making a meaningful impact throughout the rehabilitation journey? Abrazo West Campus is seeking a Part-Time Inpatient Rehabilitation Pharmacist to join our growing pharmacy team and support patients on their path to recovery.

This position offers the opportunity to work closely with rehabilitation physicians, nurses, therapists, case managers, and other healthcare professionals to optimize medication therapy and improve patient outcomes in the inpatient rehabilitation setting. Schedule includes Friday, Saturday, and Sunday coverage.

What You'll Do

As an Inpatient Rehabilitation Pharmacist, you will:

• Review, verify, and monitor medication therapy for patients in the inpatient rehabilitation unit.
• Collaborate with physicians, nurses, therapists, and case managers to optimize patient outcomes.
• Perform medication reconciliation and assist with transitions of care.
• Identify and resolve medication-related problems while promoting medication safety.
• Provide patient and caregiver medication education to support successful discharge planning.
• Participate in interdisciplinary team meetings and care conferences.
• Support formulary management, medication stewardship, and regulatory compliance initiatives.
• Assist with quality improvement activities and medication use evaluations.
• Serve as a clinical resource for medication therapy management and evidence-based practice.

What We're Looking For Required Qualifications

• Bachelor of Science in Pharmacy or Doctor of Pharmacy (PharmD)
• Current Arizona Pharmacist License
• BLS Certification
• Hospital pharmacy experience or equivalent clinical experience

Preferred Qualifications

• Doctor of Pharmacy (PharmD)
• Pharmacy Residency Training
• Board Certification (BCPS, BCGP or related specialty)
• Previous inpatient rehabilitation or acute care experience
• Experience with interdisciplinary patient care teams
