Position Purpose
The primary purpose of this position is to perform and oversee resident personal care or to assist residents in performing personal care, including dining services, in accordance with Facility policies and procedures.
Specific Duties and Responsibilities
Policies and Procedures
Maintain confidentiality of residents, resident family members, coworkers, and Immanuel Living
Understand and support the philosophy, policies, and standards of Immanuel Living and the Assisted Living Department
Maintain quality of resident care, including compliance with Facility and Assisted Living Department policies and procedures, and State and Federal guidelines
Annually review, to understand, and comply with Assisted Living Policies and Procedures and the procedures for MSDS, Exposure Control, Infection Control, and Fire Plan
Resident Care
Accurately and safely perform resident care, in accordance with residents’ service plan, resident Assignment Sheets and worksheets, and nurses’ instructions.
Follow resident Service Plans and give input for changes in writing.
Answer residents’ pendant alarms and emergency call buttons in a timely manner, then follow the incident reporting procedure.
Straighten and maintain Assisted Living area and equipment storage areas in neat and orderly condition.
Assist residents to and from meals, meetings, and activities, as needed.
Perform other duties as assigned.
Documentation and Communication
Give incoming staff a report at the end of assigned shift.
When coming on shift, check in with the Front Desk Clerk and use the communication books to update on status of residents.
Accurately record resident care on Resident Assignment Sheet.
File documentation in resident charts as needed.
Inform nurses, in writing, of improvement or decline in resident condition on a timely basis.
Charge out billable supplies.
Answer Assisted Living telephone when nurse is unavailable.
Practice appropriate phone and pager etiquette.
Communicate with supervisor and participate in training and/or meetings that help facilitate departmental communication process.
Attend and participate in in-service educational classes, on-the-job training programs, etc., as scheduled or required. Attend staff and departmental meetings, as directed.
Communicate with others in a considerate manner.
Other Duties and Responsibilities
Function effectively as a team member.
Maintain understanding, patience, and tact in dealing with residents, resident families, and coworkers, and be cooperative.
Maintain good working relationships with other staff.
Respect residents’ personal items.
Ensure maintenance of a safe living and working environment for residents and staff.
Work independently and maintain good problem-solving skills.
Maintain regular work attendance and follow established work schedule. At times, hours and days of the week must be flexible to meet scheduling requirements.

Problem Solving
Problem solving involves identification and analysis of diverse problems; answers are usually found by reviewing standard technical manuals and administrative procedures and modifying them for unusual situations. Guidance is usually provided on what sources to review and solutions are reviewed before acceptance.
Specialized Office Equipment
Operate fax machine to send or receive documents
Operate company cell phone and paging system
Operate multi-line phone to place, receive or transfer calls, or to retrieve voice mail messages.
Operate copier to copy and collate documents; paper shredder to shred
Specialized Unit Equipment
Operate lifts used in facility
Operate the medical equipment used in the department (eg. manual blood pressure cuff)
Other Required Knowledge, Skills, or Abilities
Effectively work and communicate with a wide range of individuals from all socio-economic groups, including geriatric residents, people with special needs, and those in need of memory care support
Read, interpret, and implement documents, such as governmental regulations, rules, and procedure manuals
Work with minimal supervision and effectively manage workflow
Make independent decisions when circumstances warrant such action
Respond effectively to the most sensitive inquiries or complaints from residents, resident families, coworkers, volunteers, and the public.
Essential Physical, mental, and Emotional Requirements
The employee must be able to perform multiple tasks simultaneously and work under time and/or deadline pressures.
While performing the duties of this job, the employee is frequently required to sit; use hands to finger, handle, or feel; reach with hands and arms; stoop, kneel, bend, squat, crouch, or crawl; talk and hear.
The employee must frequently lift up to 10 pounds, but rarely lift up to 20 pounds, and rarely lift up to 40 pounds.
The employee must occasionally reach above shoulder height, frequently reach from chest to shoulder, and frequently reach below waist level.
The employee must occasionally carry up to 12 pounds but rarely carry up to 25 pounds.
The employee must frequently push / pull up to 20 pounds, but rarely push up to 40 pounds and rarely push / pull up to 40 pounds.
Specific vision abilities required by this position include close vision, distance vision, color vision, peripheral vision, depth perception, and ability to adjust focus.
Education and Experience
Graduation from high school or GED preferred.
Supervision
This position operates under general supervision.
The employee’s work is primarily assigned by daily workflow and operations.
The employee is authorized to make decisions as to the timing and sequence of work and the work is performed according to established policies and procedures and/or within applicable laws, rules, and regulations governing the tasks being performed.
This position has no supervisory responsibility.

There is moderate responsibility for making recommendations or decisions which usually affect the assigned department, but may at times affect the operations, services, individuals, or activities of others outside of the assigned department.
Judgment Required in Decision-making
Varied: Supervision is present to establish general objectives relative to a specific project, to outline the desired end product and to identify potential resources for assistance. Independent judgment is required to identify, select, and apply the most appropriate of available guidelines and procedures, interpret precedents, and adopt standard methods or practices to meet variations in facts and/or conditions.
Exception Authority
This position cannot authorize exceptions to Corporate policy or procedure.
Personal Contacts
In an effective manner, this position maintains daily contact and interaction with coworkers, residents, resident families, volunteers, and the public.
This position interacts with corporate directors, managers, Nursing Services staff, residents, and resident designees, to promote resident care and health several times a month.
Working Conditions
This position works in a temperature-controlled facility environment.
This position is exposed, on a weekly or more frequent basis, to the following: biological fluids (including but not limited to vomit, urine, and feces), blood-borne pathogens, upset and/or angry residents and/or family members, risk of cuts, burns, shock, or other minor injuries (not requiring hospitalization); unpleasant substances, odors, or noises.
