Job Description

Registered Nurse

MAIN FUNCTION:

Provides nursing care to patients in an efficient, professional, compassionate and caring manner. Directs the activities of paraprofessional as they provide care. Responsible for assuring effective communication with patients, support persons and members of the healthcare team. Responsible for implementing nursing care of assigned patients via the Nursing Process including assessment, diagnosis, planning, implementation and evaluation. Acts in compliance with hospital, Nursing Department policies/procedures and the Nurse Practice Act of the State of Ohio. Follows WCH Nursing Philosophy and practice model which are derived from the Synergy model and appropriate Service Standards as outlined in Employee Handbook.

RESPONSIBLE TO: Department Director, Clinical Manager, Charge Nurses

MUST HAVE REQUIREMENTS:
  • Active license in the State of Ohio
  • Maintain a current AHA (American Heart Association) Healthcare Provider BLS card while in this position.

    • Performs assessments/reassessments and documentation relevant to patient's condition, needs and age, per nursing documentation guidelines. Communicates changes in patient condition to physician in timely manner and suggests possible interventions and course of care as appropriate in a collaborative manner.
    • Initiates, evaluates and reviews plan of care
  • Provides age appropriate health teaching related to diagnosis, identified patient/family needs and medication, using available teaching resources and assuring patient family understanding by teach back
  1. Administers medication safely:
    • Correctly demonstrates use of CPOE(Computer Physician Order Entry)
    • Demonstrates knowledge of drug purpose and effectiveness as related to diagnosis
    • Follows 6 rights of medication administration
    • Demonstrates appropriate/safe medication handling/administration
    • Reassesses and documents effectiveness of medication
  2. Assures team members huddle and round on patients to :
    • Evaluate the environment to assure safety & infection control standards are met
    • Evaluate the care provided to assure it is based on practice standards and evidence based practice
    • Evaluate the educational plan and its implementation; assessing for and addressing gaps in care
    • Evaluate for any additional /alternative care needs, and patient/family satisfaction
    • Evaluate adherence to nursing practice standards of care and documentation of care by the nurse and team members

Unit Specific Requirements:

AC / PAT/ PACU RN

MAIN FUNCTION:

 

To assess, evaluate, implement, collect data, document, monitor, administer pre and post operative medications to all surgical patients. Directs the care delivered by others in AC/PAT/PACU. Assist with physicians with procedures. Prioritize care for all types of surgical patients.

MUST HAVE REQUIREMENTS:
  • ACLS within 1 year of hire, and maintain while in position
  • PALS within 1 year of hire, and maintain while in position
  • Minimum of 1 year critical care experience
PREFERRED ATTRIBUTES:
  • Previous perioperative nursing experience
  • Ability to perform phlebotomy and IV therapy
  • Recognize cardiac arrhythmias and institute appropriate interventions
OTHER REQUIREMENTS:
  • Required to take "scheduled on call" status (PACU)
  • Must be able to respond to the hospital within 30 minutes

Cardiac Cath/Special Procedure Lab

MAIN FUNCTION:

 

Also, functions independently in the Cath Lab/Special Procedures holding room and provides nursing care in a calm, efficient manner. Admits and discharges patients. Provides pre-procedure teaching and assures physician orders are done prior to procedure. Recovers post-procedure patients according to dept policy and accurately follows physician post-procedure orders. Assists in transferring specific patients to other hospitals directly from the holding room. Provides follow-up care by phoning patients next day. Provides nursing support in other areas if needed (CT for biopsies).

Keeps patients safe & calm while assisting physicians in cardiac cath/special procedures and related interventional procedures. Functions independently in the lab while using professional judgment in performance of services. Accurately & promptly implements physician's orders. Scrubs, circulates & records all cases done in the lab & helps to recover patients. Acts in compliance with hospital policies/procedures. Provides care to patients in a calm, efficient manner.

MUST HAVE REQUIREMENTS:
  • Experience in cardiovascular medications
  • BLS and ACLS certification and maintain both while in this role
  • For new hires into the cath lab, must be able to respond to the hospital within 30 minutes
Other Position Expectations
  • Experience with hemodynamic monitoring and operation of temporary pacemakers and Intra- Aortic Balloon Pumps
  • 1 year experience in Interventional Cardiology
  • Required to scrub, circulate and monitor patients in the cath lab
  • Knowledge of coronary and peripheral anatomy
  • Knowledge of coronary and peripheral blood vessels and blood flow (venous and arterial)
  • Takes on-call status
  • Knowledge of sterile field technique
  • Recognize cardiac arrhythmias and institute appropriate interventions
PREFERRED ATTRIBUTES:
  • 1-2 years working in an interventional cardiac/peripheral cath lab OR 5 years nursing experience in ED, PCU, or ICU.
  • Cath Lab transport team experience

Cardiac Rehab Nurse

MAIN FUNCTION:

 

Collaborates with the exercise physiologist to prescribe and monitor exercise workloads and target heart rates appropriate to each individual during each exercise session. Monitors and records vital signs during exercise sessions. Communicates effectively with the healthcare team to monitor compliance and appropriately manage symptoms. Develops teaching curriculum in conjunction with Nutrition Services, Sleep Clinic, and the exercise physiologist including lectures, videos, class participation and patient educational materials.

MUST HAVE REQUIREMENTS:
  • ACLS within 1 year of hire, and maintain while in position
  • Ability to read and operate cardiac monitor, recognize normal/abnormal rhythms, and institute appropriate interventions
  • Demonstrate knowledge and use of Vasoactive medications
OTHER REQUIREMENTS:

 

  • Prescribes initial target heart rates and exercise with in conjunction with exercise physiologist
  • Ensures appropriate warm-up, exercise workloads, and cool down for each client
  • Adjusts workloads according to observed or reported symptoms, or as requested by the exercise monitor
  • Develops cardiac rehab teaching curriculum and implements lectures and materials and reviews / revises them every three months

Cardiovascular Services Registered Nurse

MAIN FUNCTION:

 

Provides nursing care to inpatients and outpatients in the Cardiovascular Services Department by working within the nursing process in an efficient, professional, compassionate and caring manner. Directs the activities of paraprofessionals as they provide care within the department. Communicates effectively with patients, support person(s) and other members of the health care team. Acts in compliance with hospital Nursing Department policies/procedures and the Nurse Practice Act of the State of Ohio.

MUST HAVE REQUIREMENTS:
  • ACLS within 1 year of hire, and maintain while in position
  • Ability to read and operate cardiac monitor, recognize normal/abnormal rhythms, and institute appropriate interventions
  • Demonstrate knowledge and use of Vasoactive medications
  • Demonstrate competency using cardiac monitor and other equipment
  • Recognize cardiac arrhythmias and institute appropriate interventions

Endoscopy RN

MAIN FUNCTION:

 

To assess, evaluate, implement, and monitor the care of patients undergoing GI procedures and those patients who are receiving Medical Outpatient Services. To assist physicians with procedures and monitor patients who are receiving moderate sedation. Prioritize the care of all patient populations.

MUST HAVE REQUIREMENTS:

1 year related medical surgical experience

BLS and ACLS maintained while in position

PREFERRED ATTRIBUTES:

Previous endoscopy experience

OTHER REQUIREMENTS:
  • Must be able to respond to the hospital within 30 minutes
  • Must be able to work flexible hours including half days
  • Cleans and checks scopes and instruments according to procedures
  • Schedules/coordinates patients and overall department schedule
  • Assist physician with Endoscopy procedures

Emergency Dept Staff RN

MAIN FUNCTION:

 

Assesses, plans, implements, evaluates and documents the care of emergently ill medical and surgical patients of all ages. Assists with special procedures. Prioritizes the care of critically ill and less acutely ill patients. Directs the care delivered by others in the ED.

PREFERRED ATTRIBUTES:

 

  • CEN (Certified Emergency Nurse)
  • TNCC ( completion of Trauma Nurse Core Curriculum)
  • Current general nursing, emergency or critical care experience. Completion of emergency or critical care course.
  • Minimum of 1 year current experience in hospital or clinical setting as an RN
OTHER REQUIREMENTS:
  • ACLS within 1 year of hire, and maintain while in position
  • PALS within 1 year of completion of orientation and maintain while in position
  • Complete NIH Stroke Certification with 6 months and maintain while in position
Additional Position Expectations
  • Demonstrates competence in patient triage using the established method, within 6 months
  • Demonstrates knowledge and use of Vasoactive medications
  • Ability to read and operate cardiac monitor , recognize normal/abnormal rhythms , and institute appropriate interventions
  • Quickly identifies signs and symptoms of Stroke and Acute Myocardial Infarction and follows appropriate protocols, policies and procedures to ensure goal times are met
  • Answers the EMS line, obtains squad report and appropriately communicates the report to the Charge RN and ED Physician
  • Demonstrates skills to assess and intervene in life-threatening situations
  • Attends in house codes, RRT and Stroke calls as required

Float Registered Nurse- Float A (Inpatient and Peds) Float B (Inpatient, Peds, and ED units) Float C (Inpatient, Peds, ED, and ICU Units).

MAIN FUNCTION:

 

Float staff will be assigned to one unit director for performance evaluation, but will also be responsible to management team where they are assigned each shift. Float staff may be pulled to different units during any given shift, depending where the staffing needs must be met. Float staff may be assigned to any area when needed and will take assignments within the scope of their skills and experience. Float nurses will be expected to take pediatric patients. In the ED, float nurses will be expected to provide care of patients from various types of illness or trauma requiring emergency attention and must be able to recognize and effectively treat life threatening conditions. In ICU, float nurses will be expected to manage critical care patients within the scope of the unit. Float staff will be expected to maintain competency in main assigned departments for their float group. Float staff will be expected to attend some of the assigned department staff meetings and/or read the meeting minutes.

MUST HAVE REQUIREMENTS:
  • ACLS within 1 year of hire, and maintain while in position
  • PALS within 1 year of hire, and maintain while in position (for Float A only)
  • Complete NIH Stroke Certification with 6 months and maintain while in position
  • Ability to read and operate cardiac monitor, recognize normal/abnormal rhythms, and institute appropriate interventions
PREFERRED ATTRIBUTES
  • ED/Critical care experience/telemetry experience.
  • IV therapy/Phlebotomy experience
  • One year current hospital nursing as RN

Home Health Registered Nurse

MAIN FUNCTION:

Provides nursing care in patients’ homes on a skilled, intermittent basis. Coordinates patient care with other disciplines, PT,OT, Home Health Aide, Speech Therapy and Social Worker to ensure that patient meets their goals. Follows Medicare Condition of Participation requirements in delivering patient care.

MUST HAVE ATTRIBUTES:
  • One year hospital nursing experience or long term care experience
  • Hold a valid driver’s license
  • Maintain car insurance
  • Flexibility with assignments and work hours based on patient’s needs

Home Health Services RN Call Nurse

MAIN FUNCTION:

Provides direct on-call coverage to home health patients under the direction and coordinator of the Clinical Coordinators. Answers all phone calls and makes decisions according to the patient’s best interest. May need to make patient visits depending on patient’s situation while on-call. Reports calls to Clinical Coordinator for follow-up.

Infusion RN

 

 

MAIN FUNCTION:

 

To assess, evaluate, implement, and monitor the care of patients receiving chemotherapy, IV infusions injections and various medical outpatient infusion needs.

MUST HAVE ATTRIBUTES:
  • BLS
  • Current Chemotherapy Bio-Course or completion within 6 months of hire
PREFERRED ATTRIBUTES:
  • Previous oncology and IV experience
  • Oncology Certified Nurse Certification (OCN)
Other Position Expectations:
  • Must be able to work flexible hours including half days and weekends as needed
  • Complete mandatory yearly competency
  • Schedule and coordinate patients and overall department schedule
  • Coordination of care with Wooster Cancer Care (WCC) staff

Inpatient Rehab RN


 

MAIN FUNCTION:

Assesses, plans, implements, evaluates and documents care of the Rehabilitation patients. Uses the Functional Instrument of Measure to develop goals for improving functional status.

PREFERRED ATTRIBUTES:
  • Recognition of positive interactions with patients and customers
  • Willingness to assist with community education
  • 1 year experience in acute rehab or acceptable related training and experience
Other Position Expectations:
  • Maintains competency in functional independence measures scoring (FIM)

  • Performs highly specialized assessments/reassessments and documentation relevant to patient's condition, needs (including pain control, and age in a policy-specified time frame

Intensive Care Registered Nurse

MAIN FUNCTION:

 

Assesses, plans, implements, evaluates and documents the age appropriate and evidence-based care of acutely and critically ill medical, cardiac and surgical adolescent/adult patients. Provides acute stroke care per protocols of the WCH designated stroke unit. Assists with special and invasive bedside procedures. Directs the care delivered by others in the department. Collaborates with Physicians to evaluate patient readiness for transfer out of critical care environment. Accepts med surg and telemetry overflow patients as necessary. Communicates effectively with the intraprofessional team and families.

MUST HAVE REQUIREMENTS:
  • ACLS within 1 year of hire, and maintain while in position
  • 1 year current hospital nursing; New graduates only with completion of critical care preceptorship or current Nurse tech position
  • Complete NIH Stroke Certification with 6 months and maintain while in position
  • Strong interpersonal skills and ability to work with a diverse community
Other Position Expectations

MUST HAVE REQUIREMENTS:
  • Cross train and maintain competency to Step-down Nursing within 1 year
  • ACLS within 1 year of hire, and maintain while in position
  • Complete NIH stroke certification within 6 months and maintain while in position
