Pathway of Hope Case Manager

Overview

GENERAL PURPOSE OF JOB.

The Pathway of Hope Case Manager provides strengths-based, client-centered case management services to families with children who are seeking to break the cycle of poverty and move toward greater stability and self-sufficiency. This position partners with households over a 6–18-month period to assess needs, develop individualized action plans, connect families to internal and community resources, and support progress toward education, employment, housing, financial stability, and overall family well-being.

Responsibilities

ESSENTIAL DUTIES AND RESPONSIBILITIES.

• Identify, assess, and enroll eligible families in accordance with Pathway of Hope eligibility criteria and established procedures.
• Provide ongoing case management services to a caseload of approximately 4-6 families, meeting with participants weekly or bi-weekly for 1-2 hour sessions over a 6-18 month period.
• Conduct intake assessments, develop individualized goal plans, monitor progress, and complete exit and post-exit follow-up sessions in accordance with program expectations.
• Introduce available pastoral care and corps-based support services to families, following established procedures when a family expresses interest.
• Assist participants with life-skill development within the Case Manager’s scope of practice, including job readiness, job search, educational advancement, parenting, budgeting, nutrition, conflict management, substance abuse awareness, and self-advocacy.
• Connect families to Salvation Army programs and external community resources, including housing, utilities, healthcare, mental health services, legal resources, childcare, public benefits, and other supports as needed.
• Support families in strengthening social support systems through community activities, schools, faith communities, and other appropriate organizations.
• Maintain complete, accurate, and timely case records in Community Services database, including assessments, social history, action plans, progress notes, referrals, follow-up contacts, and other required documentation.
• Track program statistics, participant outcomes, and contingency fund usage; prepare monthly reports in accordance with National Statistical System (NSS), NEOSA, and grant reporting requirements.
• Collaborate regularly with the Divisional Pathway of Hope Regional Coordinator and other Pathway of Hope Case Managers, including participation in monthly meetings and case consultation as requested.
• Coordinate with corps staff, divisional staff, community agencies, and service providers to support participant needs and reduce barriers to stability.
• When necessary, assist clients with rent, utility, and emergency assistance programs, ensuring appropriate documentation and compliance with funding guidelines.
• Attend community meetings or events as assigned and serve as a professional representative of The Salvation Army and the Norwalk Ohio Corps.
• Maintain confidentiality, professional boundaries, and accurate records while complying with all applicable policies, procedures, and program standards.
• Perform other related duties as assigned.

Qualifications

MINIMUM EDUCATION and/or EXPERIENCE

• Bachelor’s degree in Social Work, Human Services, Counseling, Psychology, or a related field required; master’s degree preferred.
• Prior case management experience with families, individuals experiencing poverty, or community-based social service programs is preferred. Licensed Social Worker or related credential is preferred.

SPECIAL SKILLS, CERTIFICATES, LICENSES, REGISTRATIONS

• Knowledge of case management principles, community resources, poverty-related barriers, and trauma-informed service delivery.
• Demonstrated ability to use strengths-based, client-centered approaches, motivational interviewing, and effective goal-planning techniques.
• Strong verbal and written communication skills, including accurate documentation, reporting, and professional correspondence.
• Ability to maintain confidentiality, exercise sound judgment, and build respectful working relationships with diverse individuals and families.
• Strong organizational skills with the ability to manage deadlines, caseload documentation, appointments, and grant-related reporting requirements.
• Proficiency with Microsoft Office and the ability to learn and use ServicePoint, NSS, and other required data systems.
• Valid driver’s license and ability to travel locally for meetings, community engagement, or client-related services as needed.
