Position Summary

The Registered Nurse delivers high-quality, compassionate care to people experiencing homelessness and other at-risk individuals. This role applies clinical expertise within an integrated, trauma-informed care model to meet the complex health needs of a diverse patient population. The RN promotes patient-centered care and improved well-being through adaptability, resilience, and a proactive approach to reducing barriers to healthcare.

Duties/Responsibilities (Essential Functions)

• Provides skilled nursing care consistent with community standards and CCH protocols.
• Delivers patient-centered care within an integrated, trauma-informed, culturally responsive, harm-reduction model that addresses patients' physical, emotional, and social needs.
• Conducts rapid, accurate triage assessments in person and by phone, and provides care recommendations in line with SSHC nursing protocols.
• Administers medications safely and effectively according to protocol.
• Educates and empowers patients on common chronic and acute conditions, including diabetes management, hypertension management, nutrition, wound care, infectious disease, gender-affirming care, family planning (parenting, adoption, abortion), and disease prevention.
• Oversees care for patients with transitional, chronic, and complex health conditions.
• Collaborates with the multidisciplinary healthcare team and coordinates care across internal CCH programs.
• Provides treatment services as a member of an OBH-licensed Substance Use Disorder (SUD) treatment team, including case management, crisis services, initial intake screenings, individual medication-assisted treatment (MAT) counseling, and withdrawal assessment scales.
• Mentors and precepts new nurses and nursing students to build their clinical skills and confidence.
• Facilitates patient access to internal programs, including health outreach and housing support services.
• Provides nursing services as needed across CCH programs and facilities.
• Participates in team-based and interdisciplinary quality improvement initiatives.
• Engages in ongoing professional development.
• Attends required disciplinary, interdisciplinary, suite, quality improvement, and all-staff meetings.
• Ensures timely, accurate documentation of all patient interactions, clinical services, and communications in the electronic health record (EHR).
• Performs other duties as assigned.

Education, Licensure and Certifications 

• Associate's, Bachelor's, or Master's degree in Nursing from an accredited college or university.
• Current Registered Nurse licensure in the State of Colorado.
• Basic Life Support Certification required within 30 days of employment.
• Bi-lingual English-Spanish and/or ASL preferred.

Experience

• Experience in ambulatory, public, community, mental health, emergency, or critical care nursing preferred.
• Pediatric nursing experience and familiarity working with diverse or marginalized communities are highly desirable.

• Knowledge of, and sensitivity to, the needs of people experiencing homelessness and other vulnerable populations.
• Proficiency in EHR use and documentation compliance.
• Ability to triage, assess, and provide urgent care following clinic protocols.
• Commitment to trauma-informed care, universal precautions, and regulatory compliance (OSHA, HIPAA).

• Work primarily in a clinical setting with potential for variable hours.
• Ability to work day, evening or weekend clinic as scheduled or appropriate.
