Impact Health is seeking a compassionate and dedicated Palliative Care Nurse Practitioner on a Per Diem (PRN) basis to join our growing healthcare team in Bucks County, PA!!

Compensation and Benefits:
Earning potential: $115,000–$200,000 (salary based on experience + easily achieved monthly bonuses)
Flexible Schedule: Monday – Friday. No on-call, weekends, or holidays.

Key Responsibilities
Deliver person-centered care using evidence-based practices
Collaborate with interdisciplinary facility teams
Conduct assessments, develop treatment plans, and manage follow-up care
Provide ongoing follow-up and chronic care management in Skilled Nursing Facilities (SNFs)
Educate patients, families, and staff
Complete timely documentation (within 24 hours)
Compliance with facility, state and federal regulations/protocols.
QualificationsQualifications:
Active license as a Nurse Practitioner (NP)
Minimum 2 years of post-certification experience
Experience in SNFs, hospitalist roles, emergency departments, or ICU preferred
Proficient with EHRs and clinical technology
Strong communicator with a collaborative mindset

Impact Health PC’s combined pre and post discharge focus avoids unnecessary hospital admissions to improve key quality metrics while enhancing patient and family satisfaction. Our highly skilled teams currently serve within CT, MA, NY, ME, NH and NJ.
