We’re seeking an experienced Certified Occupational Therapy Assistant (COTA) to provide PRN home health visits throughout the Winter Haven, FL area.

What You'll Do
- Provide in-home occupational therapy services under the supervision of a Registered Occupational Therapist
- Follow individualized treatment plans and physician orders
- Help patients improve daily living skills, mobility, safety, and independence
- Educate patients and caregivers on exercises, activities, and home safety
- Monitor patient progress and communicate updates with the clinical team
- Complete timely and accurate clinical documentation
What You Need
- Active Florida COTA license
- Graduate of an accredited OTA program (ACOTE)
- Current NBCOT certification or eligibility
- Minimum 1 year of home health experience required
- Strong communication and documentation skills
- Reliable transportation, valid driver’s license, and auto insurance
- AHCA Level II Background Screening required

- Sign-On Bonus Terms: The $250 sign-on bonus is available to eligible new hires who successfully complete all required onboarding requirements and remain actively employed with UPLIFT HOMECARE for 90 days. Employees must remain in good standing and meet company requirements, including timely completion of required documentation. The bonus will be paid through regular payroll following completion of the 90-day eligibility period and is subject to applicable taxes and company eligibility requirements.
