We are seeking an experienced Certified Occupational Therapy Assistant (COTA/OTA) to provide PRN home health visits in the Tampa area.

What You'll Do

As a Home Health COTA, you will:

• Provide skilled occupational therapy services under the supervision of a licensed Occupational Therapist
• Follow individualized treatment plans and physician orders
• Help patients improve activities of daily living, functional mobility, coordination, strength, and independence
• Provide therapeutic activities and exercises according to the patient's plan of care
• Educate patients and caregivers regarding exercises, adaptive techniques, home safety, and fall prevention
• Monitor patient progress and communicate changes or concerns to the supervising OT and clinical team
• Participate in coordination of care with other members of the interdisciplinary healthcare team
• Complete accurate, timely, and compliant clinical documentation
• Promote patient safety, independence, and quality of life in the home environment

What You Need

• Active Florida COTA/OTA license
• Graduate of an accredited Occupational Therapy Assistant program
• Current NBCOT certification or eligibility, as applicable
• Minimum 1 year of home health experience required
• Strong communication, organization, and documentation skills
• Ability to work independently while collaborating with the clinical team
• Reliable transportation
• Valid driver's license
• Current auto insurance
• AHCA Level II Background Screening

Compensation & Schedule

Pay: $45 per visit
Sign-On Bonus: $250

• Sign-On Bonus Terms: The $250 sign-on bonus is available to eligible new hires who successfully complete all required onboarding requirements and remain actively employed with UPLIFT HOMECARE for 90 days. The bonus will be paid through regular payroll following completion of the 90-day eligibility period and is subject to applicable taxes and company eligibility requirements.
