What the Role Looks Like
- Evaluate and treat a primarily geriatric patient population in their homes throughout Yonkers
- Develop and manage individualized plans of care with full clinical autonomy
- Help patients improve independence, safety, mobility, upper extremity function, and ADL performance
- 1:1 care every visit — no double-booking, ever
- Complete timely clinical documentation with admin team support for scheduling and logistics
- Manage your caseload based on your availability and chosen employment structure
Compensation
PRN: $72 per follow-up visit | $85 per evaluation
Part-Time W-2: $65 per follow-up visit | $75 per evaluation
Full-Time W-2 Tiered: $55–$70 per follow-up visit (increases with volume) | $75 per evaluation
Full-Time 1099 Tiered: $70–$80 per follow-up visit (increases with volume) | $85 per evaluation — earn up to $145,000 annually
Full-Time Salary: $92,000–$110,000 annually

What You'll Need
- Active NY Occupational Therapist license
- Reliable transportation
- Ability to work independently in a home-based setting
- Strong clinical, communication, and time-management skills
- Home health experience preferred but not required
