Occupational Therapist (OT) – Per Diem - Home Health Visits | North Port and Port Charlotte, FL

Matrix Home Care is seeking a licensed Occupational Therapist (OT) for an immediate PRN opportunity providing home health therapy visits in the North Port and Port Charlotte areas.

Position Details:

Location: North Port and Port Charlotte, FL
Position Type: PRN / Per Diem
Pay Rate: $70 per visit
Setting: Home Health
Start: Immediate need

Responsibilities:

Provide occupational therapy services to patients in the home setting

Evaluate patients’ functional abilities, needs, and goals

Develop and follow individualized treatment plans based on patient needs

Assist patients with improving daily living skills, safety, mobility, strength, coordination, and independence

Educate patients and caregivers on therapy goals, home safety, adaptive techniques, and recommended exercises

Monitor patient progress and communicate any changes or concerns to the care team

Complete timely and accurate documentation in accordance with company and regulatory requirements

Collaborate with nurses, therapists, physicians, and other members of the care team to support quality patient outcomes

Requirements

Active Occupational Therapist license in the State of Florida

Home health experience preferred

Strong clinical assessment, documentation, and communication skills

Reliable transportation

Ability to work independently in the field

Professional, dependable, and patient-focused approach to care

Under Florida law, this role is subject to background screening per Chapter 435, Florida Statutes, including fingerprinting and review for eligibility before employment.
