Position Summary                                                                        

The Community Health and Wellness Program Leader is responsible for planning, implementing, and  evaluating community-based programs that promote healthy aging, independence, wellness, and fall  prevention for older adults. This position provides evidence-informed fall prevention programming,  exercise and education, participant screening and functional assessments within the employee's  training and scope of practice, and wellness interventions while collaborating with community  organizations throughout Central New York. The Community Health and Wellness Program Leader  assists with program development, participant outcome measurement, data management, community  outreach, and social media engagement to support CARE's mission of improving the health, safety,  and quality of life of older adults.

This is a part time position working 20-25 hours per week. The position includes a combination of in person work at Le Moyne College and community program sites and remote work for appropriate  administrative, planning, data, outreach, and social media responsibilities.

This position is funded by external sources and is contingent on the continued receipt of those funds.

Principal Duties and Essential Functions

Job duties specific to this position:

Fall Prevention and Community Wellness

• Deliver CARE fall prevention, healthy aging, wellness, and exercise programming for older adults in community-based settings.
• Lead group exercise and educational components in a safe, engaging, and age-appropriate manner and adapt activities to participant abilities and established program protocols.
• Complete participant screenings and standardized functional measures within the employee's  education, credentials, training, and scope of practice; identify concerns that require referral to an  appropriate licensed healthcare professional.
• Provide education related to fall-risk reduction, home safety, mobility, healthy aging, assistive  devices, caregiver strategies, and other wellness topics consistent with training and professional  scope.
• Assist with the development and implementation of CARE programs, workshops, presentations,  and community initiatives.

Community Partnerships and Outreach

• Develop and maintain positive working relationships with community partners, older adults,  caregivers, healthcare organizations, and agencies serving older adults.
• Assist with outreach to identify new program sites, partnership opportunities, presentations,  community events, and referral relationships.
• Represent CARE and Le Moyne College professionally at community programs, meetings, health  fairs, and outreach events.

Social Media and Digital Outreach

• Assist with and maintain CARE's social media presence as a required responsibility of the position.
• Create, schedule, and post program updates, educational content, event promotion, participant  highlights, and community outreach content on CARE's approved social media platforms.
• Assist with basic content creation using Canva or similar tools and coordinate photographs,  graphics, captions, and program information in accordance with CARE and Le Moyne College  policies.

Data Management and Program Evaluation

• Collect, maintain, and assist with analysis of participant and program data, ensuring timely and  accurate entry into required tracking systems.
• Support outcome measurement and reporting for funders, community partners, and organizational  leadership.
• Assist with preparation of program metrics, summaries, presentations, and grant-related reports  that demonstrate program reach and impact.
• Maintain participant confidentiality and follow applicable organizational policies and data-security  practices.

General Responsibilities

• Participate in required meetings, training, and professional development activities.
• Collaborate effectively with CARE staff, Le Moyne College departments, students, community  partners, and interdisciplinary professionals.
• Demonstrate professionalism, reliability, sound judgment, initiative, integrity, and respect for the  dignity of all participants.
• Perform other duties related to CARE programming and community health initiatives as assigned.

Required Qualifications

• Education or professional preparation in a field related to health, rehabilitation, exercise, aging,  wellness, or community health. Appropriate backgrounds may include Occupational Therapy  (OT/OTA), Physical Therapy (PT/PTA), Exercise Science, Kinesiology, Health Promotion, Public  Health, Gerontology, or a closely related field.
• For applicants working under a licensed or certified healthcare profession, current New York State  licensure/certification and good standing are required when applicable to the duties performed.
• Demonstrated ability to safely lead or support exercise, wellness, health education, rehabilitation, or community-based programming.
• Strong communication and interpersonal skills and the ability to work effectively with older adults  and community partners.
• Comfort with basic technology and digital tools, including Microsoft Office, Zoom, Canva, Instagram, Facebook, and/or similar platforms.
• Ability and willingness to create and manage social media content as an ongoing responsibility of  the position.
• Strong organizational skills and ability to work independently in both in-person and remote settings.
• Reliable transportation and ability to travel to community program sites throughout Central New  York.

Preferred Qualifications

• Experience working with older adults, fall prevention, balance and mobility, community wellness,  rehabilitation, or chronic disease prevention.
• Experience leading group exercise, health education, workshops, or community-based programs.
• Experience with program coordination, community outreach, data collection, outcome tracking, or  grant-funded initiatives.
• Relevant certifications or training in evidence-based fall prevention, older-adult fitness, or wellness  programming are a plus.

Physical Requirements

The position requires the ability to travel between community sites; stand, walk, demonstrate exercise  and mobility activities, and assist with program setup; and lift/carry program materials and equipment  up to approximately 25 pounds. Work Environment and Schedule

Work is performed through a combination of community-based, on-campus, and remote settings. In person programming and travel are essential components of the role. Remote work may be used for  approved administrative tasks, program planning, data entry, outreach, social media, and other duties  that do not require on-site presence. Some schedule flexibility may be required to meet community  program and event needs.

Additional Information:

Pay Range: $35.00 - $40.00 per hour
