We are seeking an experienced Occupational Therapist (OT) to provide part-time and PRN home health services in Brandon and surrounding areas throughout Hillsborough County.

• And surrounding communities

What You'll Do

As a Home Health Occupational Therapist, you will:

• Evaluate and assess patients in their homes to determine functional needs and develop individualized treatment plans
• Provide skilled occupational therapy services according to the patient's plan of care
• Implement therapeutic interventions to improve activities of daily living, functional mobility, coordination, strength, safety, and independence
• Establish measurable therapy goals and monitor patient progress
• Modify treatment plans based on patient response and clinical findings
• Educate patients and caregivers regarding treatment plans, therapeutic exercises, adaptive techniques, home safety, fall prevention, and self-care
• Identify fall risks and recommend appropriate safety strategies and adaptive equipment when indicated
• Communicate patient progress and changes in condition with physicians and members of the interdisciplinary care team
• Maintain accurate, timely, and compliant clinical documentation
• Coordinate care with nurses, PTs, STs, CNAs, physicians, and other healthcare professionals

Requirements

• Graduate of an accredited Occupational Therapy program
• Current and active Florida Occupational Therapist (OT) license
• Minimum 1 year of home health experience required
• Current CPR certification
• AHCA Level II Background Screening
• Knowledge of applicable state and federal healthcare laws and regulations
• Strong clinical judgment and assessment skills
• Excellent communication and interpersonal skills
• Compassionate, dependable, resourceful, and patient-focused
• Ability to work independently and effectively manage time in a home health environment
• Reliable transportation
• Valid driver's license and appropriate auto insurance

• Sign-On Bonus Terms: The $250 sign-on bonus is available to eligible new hires who successfully complete all required onboarding requirements and remain actively employed with UPLIFT HOMECARE for 90 days. The bonus will be paid through regular payroll following completion of the 90-day eligibility period and is subject to applicable taxes and company eligibility requirements.
