Position Summary

As a PRN Nurse Practitioner (NP) or Physician Assistant (PA) with Impact Health PC in Lansdale, PA, you will provide weekend medical coverage to patients within skilled nursing facilities. You can provide coverage on Saturday, Sunday, or both days, based on your availability.

You will assess and manage acute and chronic conditions, develop treatment plans, provide follow-up care, and collaborate with physicians, facility staff, and interdisciplinary care teams.

Key Responsibilities

Assess, diagnose, and manage acute and chronic medical conditions.
Develop evidence-based treatment plans and provide appropriate follow-up care.
Provide weekend clinical coverage within skilled nursing facilities.
Complete timely clinical documentation and place appropriate orders.
Collaborate with physicians, nursing staff, specialists, and interdisciplinary care teams.
Educate patients and families regarding conditions and treatment plans.
Maintain compliance with facility, state, and federal regulations.

Qualifications:
Active Nurse Practitioner (NP) or Physician Assistant (PA) license in Pennsylvania.
Minimum 2 years of post-certification experience preferred.
SNF, hospitalist, emergency department, or ICU experience preferred.
Proficiency with EHRs and clinical technology.
Strong clinical assessment, communication, and organizational skills.
Ability to work independently and collaboratively as part of an interdisciplinary care team.
Availability to provide coverage on Saturday, Sunday, or both days.
