Summary
The Nurse Practitioner will manage medication refills for patients without an assigned PCP to reduce hospital and emergency room visits. They will also provide comprehensive health care, including assessment, diagnosis, and treatment, while offering telehealth support and follow-up care.
Job Description
This is an onsite part-time position 24 hours per week
The Nurse Practitioner will support the Transition of Care/ Medication Refill Team with refilling medications for patients without an assigned PCP to decrease hospital admissions and emergency room visits. Will also provide precepting by telehealth and scheduling in person/telehealth visits for those patients who need to have follow up care.
The Nurse Practitioner also will provide comprehensive, cost-effective health care to families which includes assessment, diagnosis, and treatment according to the standards of advanced nursing practice.
Educational Requirements:
Graduate of an ANA accredited Nurse Practitioner program. Master’s degree in nursing required.
Experience Required:
Minimum of 2 years of experience
Professional Licensure/Special Skills and Certification Required:
Nationally certified as a Family Nurse Practitioner
Massachusetts licensed
Current DEA/Mass Controlled Substance license for
prescriptive authority. BLS required.
Salary will be based on years of licensure
