Nurse Practitioner

Seniority Healthcare is seeking a part-time Nurse Practitioner or Physician Assistant to join our team in New City, NY for 22 hours a week. In this role, you will provide comprehensive care to a high-need, high-risk geriatric population. The ideal candidate has a background in Internal Medicine, Family Medicine, or Geriatrics, and can work independently while collaborating with a multidisciplinary team. This position offers a balance of direct patient care and remote responsibilities—such as prescription refills, lab reviews, and patient communication—allowing for a flexible and rewarding schedule.

Key Responsibilities

• Conduct thorough physical exams and wellness assessments for seniors
• Follow up on care plans to ensure the best possible outcomes
• Propose and manage treatment plans for chronic and acute conditions
• Maintain accurate patient records and manage appointments effectively
• Educate patients on preventative care, treatments, and healthy lifestyle choices
• Collaborate with a supportive team dedicated to holistic care

Qualifications

• Completion of an accredited NP or PA program
• At least one year of experience in geriatrics, internal medicine, or family practice
• Strong communication, active listening, and analytical skills
• Compassionate and committed to providing patient-centered care
• Motivated to serve a geriatric population with complex care needs
• Ability to work 22 hours a week, 2 days onsite at an assisted living facility and 3 days remote work.
