Maternity Nurse Care Manager

This position is listed on behalf of a partner company, who manages all applications and next steps. Our partner is looking for a Maternity Nurse Care Manager based in the United States.

As a Maternity Nurse Care Manager, you will provide compassionate, patient-centered support throughout the full maternity journey, from preconception and fertility planning through pregnancy, delivery, and postpartum recovery. Working primarily through telephone and digital channels, you will guide members through both routine and high-risk pregnancy experiences. You will develop personalized care plans, coordinate with multidisciplinary teams and local providers, and help members navigate complex healthcare systems and benefits. The role combines strong clinical expertise with advocacy, education, and long-term relationship building. You will play an important part in helping parents and growing families access coordinated care and achieve better health outcomes.

Accountabilities:

• Deliver coordinated, patient-centered virtual care management through telephone and messaging to support improved member health outcomes.
• Develop meaningful, individualized care plans in partnership with members and multidisciplinary care teams.
• Support members throughout fertility planning, prenatal care, delivery preparation, and postpartum recovery.
• Help members understand complex medical conditions, treatment pathways, healthcare benefits, and available resources.
• Coordinate closely with members’ local providers and internal clinical teams to promote seamless, integrated care.
• Provide compassionate longitudinal follow-up, building trusted and supportive relationships with members and their families.
• Gather relevant clinical histories, answer questions in clear and accessible language, and communicate clinical findings effectively.
• Help members navigate cultural, socioeconomic, and other factors that may affect access to care and coordinate appropriate resources.
• Maintain strict compliance with HIPAA, security, privacy, and other requirements governing protected health information.
• Collaborate effectively with clinical and non-clinical colleagues to deliver a consistent, high-quality member experience.
• Balance volume expectations and competing priorities while maintaining clinical quality, sound judgment, and attention to detail.
• Work effectively in a fast-paced, evolving remote-care environment and adapt to changing priorities and processes.

Requirements:

• Bachelor of Science in Nursing (BSN).
• Residence in a compact NLC state.
• Active Compact RN license in the state of residence, maintained in good standing with the applicable state nursing board.
• Additional active nursing licenses, particularly in Illinois, California, Minnesota, or New York, are a plus.
• Certified Case Manager (CCM) certification preferred.
• At least 5 years of nursing experience.
• At least 2 years of experience in care management, case management, or disease management, preferably within a health plan, health navigation, or third-party administrator environment.
• At least 2 years of experience working in labor and delivery.
• Strong clinical knowledge and comfort discussing a broad range of medical conditions and treatment considerations.
• Previous remote-work experience and confidence using technology and multiple computer or medical-record systems preferred.
• Highly empathetic and compassionate approach, with the ability to reassure and support patients and families during challenging circumstances.
• Strong organizational skills and the ability to work efficiently while meeting volume expectations without compromising care quality.
• Flexibility and comfort working in a rapidly changing, high-growth environment.
• Excellent verbal and written communication skills, including the ability to explain clinical information clearly to patients and summarize findings concisely for colleagues.
• Strong collaboration skills and the ability to work effectively across multidisciplinary clinical and non-clinical teams.
• Cultural awareness and the ability to recognize socioeconomic factors affecting members and connect them with appropriate resources.
• Excellent grammar, attention to detail, and the ability to communicate medical information in clear, patient-centered language.
• Commitment to maintaining confidentiality and following all applicable security and HIPAA requirements.
• Ability to remain seated and work at a computer for extended periods, with sufficient eye-hand coordination and manual dexterity to operate standard office equipment.
• No heavy lifting is generally required, although occasional exertion of approximately 20 pounds may be necessary, such as when moving a laptop or computer.
• Ability to maintain regular attendance and interact professionally and appropriately with leadership, colleagues, and members.
