Sequoia Transitional Care, a beautiful and welcoming 99-bed skilled nursing facility, is hiring a dedicated LVN Charge Nurse for our Part-Time shift. Whether you’re experienced or newly licensed, we offer a supportive environment, training, and growth opportunities to help you succeed.

Key Responsibilities:

Provide direct nursing care to residents in accordance with individualized care plans

Serve as the Charge Nurse during NOC shift, overseeing CNAs and ensuring high-quality resident care

Administer medications and treatments as prescribed
-
Monitor residents for changes in condition and report to RN Supervisor or physician
-
Document all care provided accurately and timely in EMR system
-
Communicate effectively with residents, families, and interdisciplinary team
-
Maintain compliance with safety, infection control, and facility policies
-
Respond to emergencies and follow established protocols
-
Participate in shift handoffs, training, and care planning
Qualifications:

Current California LVN license in good standing
-
CPR certification
-
Experience in a SNF or post-acute setting preferred, but not required
-

Strong written and verbal communication skills

Computer literate and comfortable with EMR systems

Compassionate, dependable, and a team player

Must be able to provide recent medical clearance to work in SNF

Must pass background check and be eligible to work in the U.S.
