The Licensed Practical Nurse renders care within their scope of practice to patients in the clinic setting providing basic patient care, monitoring vital signs, administering medications, documenting care and communicating with patients and other healthcare professionals. Works under the supervision of a provider while contributing to a collaborative healthcare team ensuring patient comfort and well-being.

Wound Clinic LPN Duties:

• Rooming

Vitals
Removal of Dressing
Clean Wound
Focused assess and measures wounds
Wound photos

• Update Provider's Orders

Fax new orders
Communicate with interdisciplinary team

• Apply Specialty Wound Dressings

Leg wraps such as unna boots and 2-layer wraps
And assist advanced therapies (wound vac, total contact cast, ultramist therapy)

• Assist Ostomy patient in applying pouching system.
• Order patient supplies through DME.
• Order clinic supplies, stock rooms and check outdates.
• Clean and turn over rooms for scheduled patients.
• Support fast paced clinic flow.
• Check voicemails and answer phone calls.
• Schedule patients and assist in managing the wound clinic schedule.

LPN Duties:

• Taking and recording vital signs (blood pressure, temperature, pulse) and observing overall patient condition. Assisting with examinations and minor procedures, collecting specimens for lab tests
• Assisting with wound care, changing bandages, and inserting catheters
• Administering certain medications, with the specific types (like IVs) dependent on state regulations and employer policies
• Maintaining accurate and detailed patient records and health charts. Managing phone calls, filing paperwork, and performing other clerical duties
• Preparing medical treatment rooms and equipment for examinations, tests, or treatments
• Maintaining an inventory of supplies and requisitioning new ones as needed
• Reporting patient status and concerns to registered nurses, doctors, and other members of the healthcare team
• Discussing care plans and medications with patients and their families advocates for patient as needed and communicates with appropriate care givers as needed
• Maintains cleanliness of workstation
• Complies with OSHA standards for safety, infection control
• Monitors equipment, supplies, medications for proper storage and expiration dates
• Participates in governance, practice, and quality initiatives
• Communicates with management as needed to support highest level of patient care
• Maintains licensure and keeps skills current
• Manages patient schedule/flow
• Interfaces with other departments in an appropriate manner to facilitate patient care
• Returns patient calls in a timely manner
• Rooms patient per department standards
• Advocates for patient as needed
• Communicates with appropriate care givers as needed

Qualifications: Minimum Qualifications

• Graduate from an accredited school of nursing with current MN license.
• CPR certification required.

Knowledge, Skills & Abilities (KSAs)

• Medical Knowledge- Understanding of disease processes, human anatomy, physiology, and pharmacology.
• Knowledge of various medical treatments, procedures, and interventions
• Understanding of infection control protocols and safety procedures.
• Attention to detail- Being precise and meticulous in administering medications, documenting information, and performing procedures to ensure patient safety and prevent errors.
• Skilled at incorporating AIDET into professional practice.
• Knowledge of role in patient confidentiality and protecting patient information from unauthorized access, use, or disclosure as defined by HIPAA.
• Ability to respond quickly and decisively to emergency situations and participate in shelter-in-place and disaster response protocols.
• Ability to work with interdisciplinary teams, communicating effectively with patients, families, and healthcare providers.
• Ability to demonstrate the knowledge and skills essential for providing care to specific patient populations.
• Ability to lift, move, and position patients and equipment.

Preferred Qualifications

• Experience as an LPN in a hospital/clinic setting
• Familiarity with EPIC electronic medical record system

Licenses & Certifications

• Licensed LPN in the state of Minnesota
• CPR certification required.

Computer Skills

Basic proficiency in Microsoft Office Suite (Word, Excel, Outlook), secure messaging applications, and collaborative tools (e.g.) Microsoft Teams, Google Workspace. Proficient with Paycom and other Information Systems applicable to position. Ability to thoroughly document clinical services, maintain accurate patient records, and effectively utilize electronic health record (EHR) systems i.e. EPIC.

Make sound decisions in real time, often under pressure (early warning signs, inconsistencies, potential complications) using critical thinking. Mathematics & Analytical Abilities

Ability to accurately perform basic mathematical calculations in all units of measure using whole numbers, common fractions, percentages, ratios, and proportions to practical situations and in analysis of quality assurance data. Ability to understand bar graphs and other visual representations of data. Accurately record and interpret clinical measurements. Use EMR systems and care documentation tools to analyze data trends and support treatment.
