Job Duties: 

Provide nursing care to patients in skilled nursing and assisted living facilities 
Administer medications and treatments as prescribed 
Monitor patient conditions and report changes to RN or provider 
Document care accurately and thoroughly in patient records 
Collaborate with care teams to deliver quality outcomes and a positive patient experience 
Ensure a safe and supportive environment for residents and staff 
 

Requirements: 

Active LPN license in the state of Wisconsin
Minimum of one year of LPN experience in a healthcare facility 
CPR certification (American Heart Association or American Red Cross) 
Strong clinical, communication, and critical thinking skills 
Reliable, professional, and able to adapt in different care settings
