What You'll Do
- Deliver skilled nursing visits including wound care, catheter care, and medication administration
- Monitor patient vitals and health status, flagging changes to the supervising RN or physician
- Educate patients and families on medications, diagnoses, and how to thrive between visits
- Implement individualized care plans under RN supervision and collaborate with the full care team
- Coordinate with RNs, therapists, and home health aides to keep care seamless and patient-focused
- Keep documentation accurate and timely — because great care deserves a great paper trail
What You Bring
- Active LPN license in the State of Florida (Compact license accepted)
- 1+ year of home health or clinical nursing experience preferred
- Current CPR/BLS certification
- Valid Florida driver's license and reliable transportation
- Proof of TB test, required immunizations, and health clearance
- Strong communication skills and a patient-first mindset
