Responsibilities

Salt Lake Behavioral Health is currently seeking a compassionate and talented Intake Assessment Coordinator (RN) to join our team.

Shift: PRN - Assist with Shift Coverage on Wednesday, Thursday, Friday, Saturday (6am-6pm)

PRN Requirements:

• Work at least 4 (12 hour) shifts or 48 hours a month
• Available to work 2 weekend or evening shifts of at least 12 hours per shift
• Work all units as needed
• Work 1 major, 1 minor holiday, and 1 hard to staff day

We believe in an individualized, patient-focused approach to diagnosis and treatment. For some, anxiety or drug dependence looks different from others with the same diagnoses, and in some cases, Behavioral health and substance use disorders occur simultaneously. In these cases of dual-diagnosis, we treat the patients in the best way possible given their specific case.

The Assessment Coordinator is responsible for providing comprehensive psychiatric and nursing assessments for individuals seeking services, ensuring timely, accurate, and clinically appropriate admission and referral decisions. The clinician serves as a frontline clinical decision-maker, conducting psychosocial and/or nursing assessments, coordinating with physicians, utilization review, business office, and external referral sources, and facilitating admissions, transfers, or community referrals across all levels of care.

Qualifications

• Education: Graduate from an accredited school of professional nursing required; Bachelor's Degree.
• Experience: 1 to 3 years experience in Behavioral Health and/or Admissions 
• Licensure: Currently licensed to practice by the Utah State Board of Nursing
• Additional Requirements: Thorough knowledge of Joint Commission, HCFA, OSHA requirements, and state and federal regulatory requirements. CPR certification and successful completion of training in Handle With Care within 90 days of employment, and prior to assisting in restraining procedures.
