Position Summary
The Social Worker supports patients in meeting their psychosocial needs and achieving emotional health. They facilitate treatment planning, conduct assessments, provide psycho-education, offer short-term therapy, lead group sessions, and coordinate discharge planning with community resources. Using evidence-based practices, they help clients reach their optimal mental and emotional well-being.
Required:
Licenced Agency Affiliated Counselor (AAC), or Social Worker Associate Independent Clinical (LSWAIC)
BLS, First Aid, Handle with Care, Verbal De-escalation (INBH Provided)
Preferred: Licensed Mental Health Counselor (LMHC), Licensed Independent Clinical Social Worker (LICSW), Licensed Marriage and Family Therapist (LMFT), Licensed Advanced Social Worker (LASW), Licensed Psychologist
Qualifications
Essential Duties & Responsibilities
- Conduct psychosocial assessments to evaluate patients' emotional, behavioral, developmental, social, and support needs.
- Provide individual, family, and group interventions to support treatment goals and patient progress.
- Develop, implement, and update individualized treatment plans in collaboration with patients, families, and the interdisciplinary treatment team.
- Facilitate treatment planning meetings and coordinate care with physicians, nursing staff, and other clinical disciplines.
- Assist patients and families with identifying resources and addressing psychosocial, educational, financial, and discharge planning needs.
- Monitor patient progress, evaluate treatment effectiveness, and adjust interventions as appropriate.
- Facilitate family meetings and support ongoing communication between patients, families, and the treatment team.
- Maintain timely, accurate, and compliant clinical documentation, including assessments, treatment plans, progress notes, and discharge planning activities.
- Ensure confidentiality and compliance with all applicable regulatory, accreditation, and organizational standards.
