         and developmental disabilities and their families ·        Facilitates linking, advocating, coordinating, and monitoring services; to assist consumers 
         in gaining access to health services, financial assistance, employment, education, social 
         services, and natural supports ·        Assesses needs, develops, and monitors Person Centered Plans and documents treatment  Qualifications:     ·        Bachelor in Social Work or Master's Degree in an appropriate mental health discipline·        QIDP, one years’ experience providing treatment to individuals with Intellectual and 
         Developmental Disabilities·        Must have reliable means of transportation for home visits and maintain Safe Driving Permit
