Hospice Weekend Registered Nurse (RN)

We are seeking a compassionate and dependable Registered Nurse (RN) to join our hospice team and provide skilled nursing support to patients and families during after-hours and weekend shifts.

Compensation

RN: $39.90–$44.23 per hour

Position Summary

The Hospice RN works collaboratively with the triage nurse and interdisciplinary hospice team to provide skilled nursing visits to patients and families requiring after-hours support.

This position responds to patient needs in the home or care setting, assesses changes in condition, provides symptom management and education, communicates with providers and the hospice team, and helps ensure patients and families receive timely and compassionate care.

Saturday and Sunday shifts are available.

Qualifications

• Graduate of an accredited school of nursing
• Current and unrestricted Minnesota Registered Nurse (RN) license
• Minimum of three years of recent nursing experience in acute care, long-term care, home care, hospice, or public health preferred
• Strong patient assessment, care planning, and patient/family education skills
• Excellent written and verbal communication skills
• Strong critical-thinking and clinical decision-making abilities
• Ability to work independently while collaborating effectively with the interdisciplinary team
• Current Minnesota driver’s license and automobile insurance
• Daily access to a dependable vehicle for patient visits
• Basic computer proficiency and ability to complete electronic clinical documentation
• Compassionate, dependable, and comfortable supporting patients and families during end-of-life care

Essential Responsibilities

• Provide compassionate, respectful, and professional care to patients and families.
• Collaborate closely with the triage nurse and complete face-to-face patient visits as directed based on clinical needs.
• Assess patients for changes in condition, symptom-management needs, and other clinical concerns.
• Provide skilled nursing interventions to promote comfort and quality of life.
• Communicate significant findings, changes in condition, patient and family concerns, and changes in the care environment to the physician, Clinical Manager, and other members of the hospice interdisciplinary team.
• Obtain and implement physician orders as appropriate.
• Educate patients, families, and caregivers regarding symptom management, medications, comfort measures, and the hospice plan of care.
• Complete clinical documentation accurately and within established timeframes.
• Work collaboratively with other hospice team members to ensure continuity and quality of care.
• Maintain patient confidentiality and comply with HIPAA requirements.
• Demonstrate knowledge and understanding of patient rights, vulnerable adult requirements, and hospice standards of care.
• Demonstrate sound clinical judgment and effective decision-making in urgent or changing patient situations.
• Seek and accept feedback regarding job performance and opportunities for professional growth.
• Consistently provide high-quality, patient-centered end-of-life care.
