Qualifications & DutiesYour responsibilities include:

Develop and implement individualized care plans based on current and prior medical history
Deliver attentive, one-on-one support to patients and their families, ensuring comfort, dignity, and understanding
Conduct thorough assessments of physical, emotional, and spiritual needs throughout the care journey
Accurately monitor and document vital signs including temperature, pulse, respiration, and blood pressure
Administer physician-ordered medications and treatments with skill and empathy
Educate patients and their families on care techniques and symptom management
Track and communicate changes in patient condition, ensuring timely and effective responses
Maintain detailed and accurate clinical documentation reflecting patient progress and quality of care
Collaborate with interdisciplinary team members to ensure comprehensive care delivery
Perform additional duties as assigned in support of patient care and team success

What You Bring:

A heart for hospice care and a commitment to exceptional service
A valid Registered Nurse (RN) license in good standing in the state where you will practice
Current CPR certification
A valid driver's license and reliable transportation
Strong observational, documentation, and communication skills
A flexible, collaborative, and patient-focused approach to care
