Pay Rates – Per Completed Visit
Per Visit - $90
Downtime (pre-approved) - $60/hour
"Completed" = visit done in the home + documentation finished within 24 hours + orders/signatures uploaded + meets QA standards.

What You'll Do
- Conduct face-to-face hospice eligibility visits and complete required documentation in compliance with Medicare hospice regulations
- Perform comprehensive assessments of hospice patients in their homes, including symptom management evaluation and goals-of-care discussions
- Collaborate with the attending physician and interdisciplinary hospice team to develop and adjust individualized plans of care
- Prescribe and manage medications focused on comfort, pain management, and symptom control
- Provide advanced clinical guidance to hospice field staff including RNs, LPNs, and aides
- Educate patients and families on disease progression, comfort-focused care, and end-of-life planning
- Participate in interdisciplinary team meetings and case conferences as needed
- Maintain accurate and timely documentation in the EMR within required timeframes
- Ensure compliance with Medicare, Medicaid, and CHAP hospice standards

Requirements
Must-Have:
- Active NP license (Ohio) + current BLS certification
- Master's degree in Nursing (MSN) or Doctor of Nursing Practice (DNP)
- Active DEA registration
- Reliable vehicle and valid driver's license with ability to travel throughout the Cleveland / Northeast Ohio service area
- Minimum 2 years of advanced practice nursing experience
- Familiarity with Medicare and Medicaid hospice regulations
Preferred / Nice-to-Have:
- Prior hospice or palliative care NP experience
- ACHPN certification or willingness to pursue
- Experience conducting face-to-face hospice eligibility visits
- Familiarity with HCHB or similar home health/hospice EMR
- Strong goals-of-care conversation and family communication skills
