Position Summary
Sunrise Home Health is seeking a compassionate and dedicated Speech-Language Pathologist (SLP) to join our growing team. The Speech Therapist is responsible for evaluating and treating patients with speech, language, cognitive, communication, and swallowing disorders in the home setting. This role focuses on improving patients’ independence, safety, and overall quality of life through individualized treatment plans and collaborative care.
Essential Responsibilities
Evaluate patients to identify speech, language, cognitive, communication, and swallowing deficits.
Develop individualized treatment plans based on physician orders and patient goals.
Provide therapeutic interventions to improve communication abilities, cognitive functioning, and swallowing safety.
Educate patients, families, and caregivers on treatment strategies and home exercise programs.
Collaborate with physicians, nurses, therapists, and other members of the interdisciplinary care team.
Maintain accurate and timely clinical documentation in accordance with Medicare and agency guidelines.
Participate in care conferences and contribute to patient care planning.
Ensure compliance with all agency policies, state regulations, and professional standards.
