About the Role:Project New Generation LLC is looking for a dedicated and experienced Home Health Service Director RN to lead and elevate our home health operations in Glendale, AZ. Responsibilities:
- Oversee and manage day-to-day clinical operations of the home health agency
- Ensure compliance with state, federal, and Medicare/Medicaid regulations and standards
- Supervise, mentor, and support clinical staff including RNs, LPNs, and home health aides
- Conduct patient assessments, care plan development, and ongoing case management oversight
- Monitor and maintain quality assurance and performance improvement initiatives
- Collaborate with physicians, patients, and families to coordinate comprehensive care plans
- Manage staffing, scheduling, and clinical documentation to ensure regulatory compliance
Requirements:
- Active Arizona Registered Nurse (RN) license in good standing
- Minimum 2+ years of home health or clinical management experience
- Strong knowledge of Medicare/Medicaid home health regulations and CMS guidelines
- Experience with OASIS documentation and home health billing processes preferred
- Proven leadership, communication, and team-building skills
- CPR/BLS certification required
- Reliable transportation and valid driver's license for occasional field visits
